  • The role of the nurse in caring for the critical patient with sepsis

    OBJECTIVES: to know the nursing interventions in the identification, prevention and control of sepsis in critical patients. METHODS: integrative review of the literature, with two parallel researches using different MesH terms, using the EBSCO database and Google Scholar. Nine studies were included in the sample. RESULTS: nursing interventions are centered on the creation/implementation of protocols for the early recognition of sepsis, the training of teams to ensure a safe and effective approach and the adoption of measures for infection prevention and control as a way to prevent sepsis. FINAL CONSIDERATIONS: the evidence shows that nurses are fundamental in the early identification, control and prevention of sepsis, preventing disease progression and contributing to decreased morbidity and mortality.
  • Stabilization of the spine in the trauma victim - integrative review

    Goal: To identify harmful effects caused by the stabilization of the vertebral column in a trauma victim and in trauma situations without indication for stabilization of the spine in the prehospital. Method: It was perform an integrative literature review guided by research questions: is there a scientific evidence of harmful effects on trauma victims caused by spinal stabilization in prehospital care? and are there situations of trauma with no indication for stabilization of the spine? Results: We have performed a Boolean search in the electronic bases Cochrane Library and Pubmed and through the EBSCOhost engine in the databases CINAHL Plus, MEDLINE, MedicLatina, SPORTDiscus, PsycARTICLES, PsycBOOKS, Psychology and Behavioral Sciences Collection, Academic Search Complete. Twelve articles were obtained and after applying the inclusion and exclusion criteria, the sample was five articles. Conclusions: Harmful effects of spinal stabilization on the victim of trauma related to airway management, pain, discomfort and pressure injuries are described. Situations of penetrating trauma with unstable circulation and victims with gunshot injuries to the head do not require stabilization of the spine. Recommendations to support the prehospital decision regarding stabilization of the spine were collected. It is crucial for the improvement of prehospital care to integrate an individualized approach of the victim that refers to its clinical state and mechanism of injury.
  • Nursing approach to the person in critical situation submitted to extracorporeal membrane oxygenation: scoping review

    Extracorporeal Membrane Oxygenation (ECMO) corresponds to a form of mechanical cardiopulmonary support used to treat severe respiratory failure, since it allows extracorporeal gas exchange. Additionally, in patients with severe heart failure, or experiencing cardiorespiratory arrest, this technique also provides systemic circulation. Nevertheless, ECMO carries some risks, such as clot formation. It is in this context that the nurse has a key role. Objective: To map the available scientific evidence about the nurses' approach to the critically ill person subjected to ECMO. Method: We conducted a scoping review, using databases accessed through the EBSCOhost platform and following the methodology proposed by the Joanna Briggs Institute. Through the combination of appropriate MesH descriptors, we selected articles published between 2009 and 2019, written in Portuguese or English. Results: Five studies were included in the review. Their analysis allowed us to verify that the nurse's approach focuses on the monitoring, surveillance, management and coordination of the care provided to critically ill patients subjected to ECMO. It was also found that the adoption of adequate protocols, the training of the multidisciplinary teams and an efficient communication between team members contributed to an effective, safe and high-quality performance when facing patients subjected to ECMO. Conclusion: An appropriate nursing approach is essential to ensure the provision of care with quality, and safety, to the patient subjected to ECMO.
  • Checklist for a safe intra-hospital transport of critically ill patients: a scoping review

    Transporting critically ill patients is has a set of risks that may jeopardize their safety. Knowing the risks associated with intrahospitalar transport of critically ill patients is essential to improving patient safety. For the sake of improving patient safety, was chosen to approach it as an intrahospitalar transport checklist, as the literature describes it as a practical and simple way to increase safety. Objective: Map available scientific evidence regarding aspects of a checklist that ensure the safety of critically ill patients in intrahospitalar transport. Material and Method: A scoping review was performed following the methodology proposed by The Joanna Briggs Institute in databases using the EBSCOhost and B-on search engines. Conducted research in Portuguese, English, and Spanish. Selected free full text articles, with no time limit. Inclusion criteria: adult/elderly critically ill patient, intrahospital transport, a checklist and patient safety. Results: Included 7 articles for analysis. Most verification requests that ensure the safety of critical patients on intrahospitalar transport concern the transport preparation phase, in which patient is monitored was the most mentioned aspect. From the post-transportation phase, checklist aspects are only available in 3 of 7 articles Conclusion: All studies address aspects that improve the safety of critically ill patients in intrahospitalar transport and are likely to be included in the checklist. There is no unanimity as to which aspects to include in the checklist.
  • Cuidados de enfermagem podológicos promotores de conforto ao idoso: uma revisão integrativa

    Introdução: Os cuidados podológicos referem-se aos cuidados com as extremidades dos membros inferiores e pés, nomeadamente na avaliação dos pés da pessoa, higiene e intervenções que abordam problemas com a manutenção ou recuperação de problemas minor das unhas e pés. Envolvem um conjunto de intervenções não invasivas que protegem a saúde dos pés e que devem constituir um foco de atenção/intervenção dos enfermeiros. Objetivo: Identificar na literatura científica as alterações que ocorrem nos pés dos idosos e conhecer os cuidados de Enfermagem podológicos promotores de conforto ao idoso. Materiais e Métodos: Trata-se de uma revisão integrativa da literatura realizada em fevereiro de 2020 nas bases de dados científicas: CINAHL; MEDLINE Cochrane e Mediclatina, no período amostral de janeiro 2009 a dezembro 2019. Resultados: Nos 27 artigos encontrados foram selecionados 5, que evidenciaram as alterações que ocorrem nos pés dos idosos e respetivos cuidados de enfermagem podológicos. Estes cuidados devem, para além de confortadores, avaliar e identificar os problemas, evitar as incapacidades e as complicações, reabilitar as incapacidades detetadas e promover a independência funcional, o autocuidado e a autoestima, levando a uma melhor qualidade de vida do idoso. Conclusão: A prevalência de problemas nos pés dos idosos é uma realidade. Foram apresentadas as alterações que ocorrem nos pés dos idosos, bem como as intervenções de enfermagem que respondem às necessidades podológicas e de conforto.

    O tema «Do Cuidado em Enfermagem: Uma nova coreografia do saber» convoca-nos, numa ligação ao lema anual da UCP e sustentados na sua missão, para um dia de «encontro», de diálogo transversal de saberes, em que buscaremos despertar para a harmonia do cuidado, enquanto compromisso comum e integral, que procura, perante os problemas da humanidade, preparar profissionais qualificados para dar respostas consistentes e integradoras, tendo como finalidade um cuidado mais responsável e sustentado, fundado no reconhecimento da dignidade e humanidade da pessoa humana. A vida humana é vida de cuidado, e uma sociedade que cuida tem de ter em si, justamente, o sentido da humanidade, associado à responsabilidade do cuidar, acentuando em particular o papel da educação. O modo de ser-no-mundo dá-se na forma de interação e de intervenção, e realiza-se pelo cuidado. Em época de desafios constantes, desejamos que as reflexões acerca «Do Cuidado em Enfermagem: Uma nova coreografia do saber» elevem o processo de intervenção no campo interdisciplinar da saúde, em prol da vida da pessoa que se manifesta, que se dá que se partilha, que se confia.
  • Global comfort scale: construção e validação de conteúdo

    Introdução: O conforto é um indicador de boas práticas pelo que é fundamental a sua promoção e avaliação, qualquer que seja a condição de saúde/doença da pessoa.Objetivo: Construir e validar o conteúdo da Global Comfort Scale.Métodos:Estudo metodológico. A validação de conteúdo foi realizada com recurso à técnica de e-Delphi com peritos que se pronunciaram acerca da clareza, pertinência e relevância. A validade de conteúdo dos itens foi verificada através do IVC-I e da escala através do IVC-S. O pré-teste efetivou-se por reflexão falada a 20 elementos da população alvo (pessoas adultas ou idosas, saudáveis ou em situação de doença) e aplicou-se a versão final do instrumento a 43 participantes.Resultados:12 peritos avaliaram o conteúdo dos itens do instrumento através de uma escala de resposta tipo Likert, tendo resultado numa escala final constituída por 55 itens com um critério de aceitabilidade superior a 0,80. Obteve-se um IVC-S relativo à clareza de 0,97; à pertinência de 0,90 e à relevância de 0,93, o que confirma a validade do conteúdo. Na reflexão falada, os participantes não tiveram dificuldades nem sugestões. Os 43 participantes, apresentam um bom nível de conforto em todos os itens. Verificámos um alfa de Cronbach de 0,981 e valores de correlação de Pearson do item com o total da escala sem o item entre 0,247 e 0,879.Conclusão:O instrumento apresentou bom índice de validade de conteúdo, seguindo-se numa fase posterior a validação psicométrica.
  • Palliative care for patients with heart failure in critical care: scoping review

    Introduction: Heart failure is a chronic condition affecting thousands globally. At some stage of their illness, these individuals require critical care. Palliative care is essential throughout the course of the disease. Objective: To map the evidence on palliative care for people with heart failure in critical care. Methods: A scoping review was conducted following the methodology proposed by the Joanna Briggs Institute. Searches were performed in electronic databases: MEDLINE Complete (via EBSCO), CINAHL (via EBSCO), PubMed, Web of Science, and Repositórios Científicos de Acesso Aberto de Portugal (RCAAP, via b-on). Results: 10 sources of evidence were included in this review. The results were grouped into 2 categories-palliative care in the emergency department and palliative care in the intensive care unit. Eight subcategories were identified: identification of the need for palliative care; symptom control; communication, decision-making and advance care planning; management of implantable cardiac devices and ventricular assistance; the dying process; culturally sensitive care; definition of prognosis; bioethical principles and informed consent. Conclusion: Critical care settings are the places where people with heart failure turn to for help with their illness. There are specific interventions for each context that can increase people's quality of life.
  • Embracing time: the (dis)comfort of life after allogeneic stem cell transplants

    Allogeneic Stem Cells Transplantation (ASCT) and, consequently, the chronicity associated with this life event has a growing prevalence and a significant impact on the life and daily life of each person who experiences it. The growing number of ASCT survivors highlights the need to reflect on the unique challenges they face throughout their recovery. To understand the meaning of the encounter with time in the lived experience of comfort for survivors of ASCT. Qualitative approach using van Manen's phenomenology of practice. We uncovered the phenomenon through phenomenological interviews, which integrated narratives and illustrative episodes that reflected the lived experiences of 20 survivors. Descriptions of lived experiences were collected from participants between July 2020 and May 2021. The following themes emerged from the ASCT survivor's lived experience of comfort related to time: giving time to live, time for what is important, time for the future, and living the time of day. This study reveals the importance of time for the survivor, the anguish of the time of discomfort, and creativity in the search to live the time of comfort provided by the ASCT.
