a prótese parcial removível é uma das soluções para reabilitar a cavidade
oral de pacientes parcialmente desdentados. Na fase pré-protética devem-se preparar os
nichos na superfície do dente pilar, pois permitirão a adaptação do apoio oclusal para
um correto suporte da prótese em boca. Contudo, a execução do nicho é considerada por
muitos profissionais como uma iatrogenia, suscetível a lesões de cárie.
Objetivos: avaliar a prevalência de cáries dentárias nos nichos dos dentes pilares das
próteses parciais removíveis. Verificar se existe relação entre os nichos protéticos, cárie
nos dentes pilares e não pilares, índice de placa e prematuridade/interferência no apoio.
Determinar se existe uma diferença significativa entre os nichos e os restantes dentes
homólogos/antagonistas da cavidade oral, ao nível da presença de cárie dentária.
Material e métodos: foram contatados, para efetuarem uma consulta de controlo, os
pacientes que colocaram próteses parciais removíveis esqueléticas na Clínica
Universitária da UCP-Viseu, entre 2010 e 2013. Numa ficha clínica especificamente
preparada para estas consultas, foram registados dados referentes à cavidade oral e à
prótese. A análise estatística foi efetuada com recurso aos testes t de Student,
coeficiente de correlação de Pearson e teste de Fisher (p<0,05).
Resultados: a taxa de adesão foi de 35,6%. Nos 53 pacientes analisados, a média de
idades foi de 60 anos. Cerca de 62,3% dos casos estudados apresentavam reabilitação
bi-maxilar. O índice de placa dos dentes pilares (1,49), foi maior, comparativamente ao
dos não pilares (1,31). Dos 255 nichos avaliados, 23 apresentavam cárie. Verificou-se
que havia uma relação significativa, entre o número de nichos e o número de cáries.
Não se verificou associação entre as cáries nos nichos, o índice de placa dos dentes
pilares, prematuridade/interferência no apoio, índice de cariados, perdidos e obturados
dos dentes pilares, bem como no parâmetro cárie dos dentes não pilares.
Conclusão: dentro das limitações deste estudo, podemos concluir que os nichos
protéticos não apresentam mais lesões de cárie dentária do que os restantes dentes da
cavidade oral. Contudo, quanto maior o número de nichos de uma reabilitação, maior a
possibilidade de se verificarem lesões de cáries.
Introduction: the removable partial denture is one of the solutions to rehabilitate the oral cavity from partially edentulous patients. In the pre-prosthetic stage, the rest seat is prepared in the surface of the abutment tooth, which will allow the adaptation of the occlusal rest for a correct support of the denture. The implementation of the rest seat is though considered, by many professionals, as an iatrogenic, leading to caries lesions. Objectives: to evaluate the prevalence of dental caries in rest seats of abutments teeth from partial removable dentures. To check whether there is a relationship between the prosthetic rest seats, caries in the abutment and non-abutment teeth, plaque index and prematurity or interference in the occlusal rest. To determine if there is a significant difference between the presence of dental caries in rest seat in comparison with other homologous/antagonists teeth in the oral cavity. Material and methods: the patients, who were placed in skeletal removable partial dentures at University Clinic of Catholic University-Viseu between 2010 and 2013, were contacted to make a control visit. Data regarding the oral cavity and the prosthesis were collected and recorded in a specifically prepared protocol. Statistical analysis was performed by using the t test Student, Pearson correlation coefficient and Fisher (p<0.05). Results: the accession rate was 35.6%. The average age of the 53 analyzed patients was 60 years old. About 62.3% of the evaluated patients had bi-maxillary rehabilitation. The plaque index of the abutment teeth (1,49) is bigger when compared to the nonabutment teeth (1,31). From the 255 rest seat evaluated, 23 had dental caries. It was found that there was a significant relationship between the rest seat and the number of dental caries. There was no association between caries in rest seats, the plaque index on abutment teeth, prematurity/interference in the support, dentition status of abutment teeth as well as in the caries parameter of non-abutment teeth. Conclusion: within the constraints of this study it can be concluded that prosthetic rest seats do not have more dental caries lesions that the remaining teeth of oral cavity. However, the bigger the number of rest seats of a rehabilitation, the bigger the possibility of occurring caries lesions.
