O presente relatório surge no âmbito do 13º Mestrado em Enfermagem, na Especialização em Enfermagem Médico-Cirúrgica na área de Enfermagem à Pessoa em Situação Crítica, do Instituto de Ciências da Saúde da Universidade Católica Portuguesa, no ano letivo 2020/2021. O documento pretende, de forma crítica e reflexiva, descrever as competências desenvolvidas como enfermeira especialista em enfermagem médico-cirúrgica ao longo das unidades curriculares de estágio. O relatório encontra-se dividido por capítulos, sendo os seus principais temas a competência cultural dos enfermeiros de cuidados críticos, apresentado sob a forma de uma revisão scoping com base nas recomendações do The Joanna Briggs Institute, e o desenvolvimento de competências em contextos de estágio, onde é descrita e analisada a experiência nos diversos contextos de intervenção, assim como as competências desenvolvidas durante este processo. Ao longo do desenvolvimento destas competências em contexto de estágio foram identificadas algumas necessidades e elaborados trabalhos com o objetivo de promover a prática baseada na evidência e contribuir para a melhoria da qualidade dos cuidados. No contexto de Serviço de Urgência, foi possível identificar uma necessidade de uniformização da abordagem à pessoa vítima de trauma, tendo sido realizado um protocolo com o objetivo de padronizar esta abordagem. No contexto de Cuidados Intensivos, foi identificada uma necessidade de realização de treino de colocação e remoção de equipamentos de proteção individual no âmbito da pandemia por SARS Cov-2. Neste sentido foram criadas grelhas de observação para orientar o treino dos profissionais na utilização de destes equipamentos, com o objetivo de minimizar o contágio e disseminação deste vírus. Ainda em contexto de cuidados intensivos, num local de estágio diferente, foi realizada uma revisão bibliográfica com o objetivo de identificar as intervenções de enfermagem para a prevenção de úlceras por pressão associadas às interfaces da ventilação não invasiva. Os resultados dessa revisão foram divulgados à equipa através de um póster. Este percurso traduziu-se numa mais valia no enriquecimento de conhecimento e desenvolvimento de competências na prestação de cuidados diferenciados à pessoa em situação crítica, numa abordagem holística da pessoa com intervenções de maior qualidade.
This report comes under the scope of the 13th Master's Degree in Nursing, in the Specialization in Medical-Surgical Nursing in the area of Nursing for People in Critical Situation, from the Institute of Health Sciences of the Catholic University of Portugal, in the academic year 2020/2021. This document is intended, in a critical and reflexive way, to describe the skills developed as a specialist nurse in medical-surgical nursing throughout the clinical teaching curricular units. The report is divided into chapters, the main themes of which are the cultural competence of critical care nurses, presented in the form of a scoping review based on the recommendations of The Joanna Briggs Institute, and the development of skills in internship contexts, where the experience in the different intervention contexts is described and analysed, as well as the skills developed during this process. Throughout the development of these skills in the context of an internship, some needs were identified and work was carried out with the objective of developing evidence-based practice and contributing to improving the quality of care. In the context of the Emergency Service, it was possible to identify a need to standardize the approach to the trauma victim, having carried out a protocol aiming to standardize this approach. In the context of Intensive Care, it has also been identified a need to perform training sessions for the placing and removing personal protective equipment within the scope of SARS-Cov-2. In this sense, observation grids were created to guide the training of professionals in the use of this equipment, with the aim of minimizing the contagion and spread of this virus. Still in the context of intensive care, at a different stage, a bibliographic review was carried out in order to identify nursing interventions for the prevention of pressure ulcers associated with non-invasive ventilation interfaces. The results of this review were shown to the team through a poster. This path has translated into an added value in enriching knowledge and developing skills in providing differentiated care to people in critical situations, in a holistic approach to the person with higher quality interventions.
