| dc.description.abstract | O documento exposto pretende descrever o percurso de aprendizagens e aquisição de competências no âmbito do 10º Curso de Mestrado com Especialização em Enfermagem Médico-Cirúrgica, conforme preconizado pela Universidade Católica Portuguesa – Instituto de Ciências da Saúde, incidindo sobre a Unidade Curricular - Estágio em Enfermagem Médico-Cirúrgica, que decorreu cronologicamente num Serviço de Cuidados Paliativos (Módulo III), num Serviço de Urgência (Módulo I) e numa Unidade de Cuidados Intensivos (Módulo II). Através de uma metodologia descritiva e crítico-reflexiva abordam-se os objetivos de estágio, atividades na consecução dos mesmos, com orientação pelos domínios da responsabilidade profissional, ética e legal, melhoria de qualidade contínua e aprendizagens profissionais. Refletem-se cuidados especializados à pessoa em situação crítica, englobando a avaliação diagnóstica, concreta e em tempo útil, por forma a reconhecer, prever e detetar precocemente as complicações decorrentes do processo de doença e conduzindo à readaptação total do indivíduo. Os cuidados especializados à pessoa em situação paliativa decorrem de um controlo de sintomatologia eficaz, no suporte psicológico, emocional e espiritual, com valorização da comunicação, no cuidado à família e no trabalho em equipa. Descreve-se o conceito de vulnerabilidade da pessoa doente e da família, alertando para os fatores que o potenciam e que são alvo de intervenção do Enfermeiro Especialista em Médico-Cirúrgica. As oportunidades de melhoria encontradas ao longo do percurso académico enriqueceram o presente documento, no sentido de as perceber e solucionar. No Serviço de Cuidados Paliativos promoveu-se a reflexão sobre o apoio à família no processo de perda, inerente à morte, com a realização de uma proposta de protocolo de acompanhamento no luto que se coaduna com a qualidade dos cuidados, nomeadamente com o envio da “carta de luto”. No Serviço de Urgência respondeu-se à necessidade de um manual de integração e acolhimento dos Enfermeiros, como parte integrante de uma gestão eficaz. Na Unidade de Cuidados Intensivos desenvolveu-se um estudo de caso – Refletir e Agir sobre a Vulnerabilidade do Doente Crítico que permitiu o enriquecimento acerca do conceito, sobressaindo na aprendizagem profissional. | dc.description.abstract | The present document intends to describe the course of learning and acquisition of competences in the ambit of the 10th Masters´ Course with a Specialization in Medical- Surgical Nursing, as recommended by the Universidade Católica Portuguesa – Health Sciences Institute. It pertains to the Curricular Unit – Internship in Medical-Surgical Nursing, which chronologically occurred in a Palliative Care Service (Module III), na Emergency Department (Module I) and in an Intensive Care Unit (Module II). Through a descriptive and critical-reflective methodology, we detail the internship goals, and all activities in the accomplishment of the former, through guidance in the fields of professional, ethical and legal responsibilities, as well as continuous quality improvement and professional learning. Specialized care is considered for a person in critical condition, encompassing the concrete and timely diagnostic evaluation, in order to recognize, predict and detect early complications arising from the disease process and leading to the complete readaptation of the individual. Specialized care for the person in a palliative condition results from an effective symptom control, as well as psychological, emotional and spiritual support. Another key element is communication, both in family care and teamwork. The concept of vulnerability of the sick person and the family is described, alerting to the factors that empower it, and which are the target of the intervention by the Medical- Surgical Specialist Nurse. The improvement opportunities found throughout the academic course have enriched thepresent document, in order to understand and solve them. In the Palliative Care Service, a reflection on support for the family in the loss process inherent to death, was promoted, with a proposal for a follow-up protocol in mourning. This is in line with the quality of care, namely by sending a “condolence letter”. In the Emergency Department, a manual of the integration and welcoming of nurses was needed as an integral part of na ffective management. In the Intensive Care Unit, a case study was developed – Reflect and Act on Critical Illness Vulnerability that allowed the enrichment of the concept, emphasizing professional learning. In conclusion, the initial purpose is considered to be achieved, although there is still work to be further developed. | pt_PT |
