O compromisso da classe profissional na procura contínua e sustentabilidade dos saberes da enfermagem prevê uma assistência de qualidade, segura e digna, fundamentada numa prática baseada na evidência. Integrado no Curso de Mestrado em Enfermagem com Especialização em Enfermagem Médico-Cirúrgica, na área da Pessoa em Situação Crítica do Instituto de Ciências da Saúde – Porto da Universidade Católica Portuguesa, o presente relatório visou a exposição das atividades e situações de maior relevância na aquisição e sedimentação de competências do Enfermeiro Especialista no desenrolar dos Estágios realizados em contexto de Urgência e de Unidade de Cuidados Intensivos Monovalente, durante o período de 9 de Setembro de 2019 a 8 de Fevereiro de 2020. Neste sentido, o presente relatório foi organizado de forma a, numa primeira fase, dar a conhecer o contexto de prática clínica onde se desenvolveu a Unidade Curricular A pessoa em situação crítica e família: vigilância e decisão clínica, visando realçar as competências aí adquiridas, nomeadamente comunicacionais e relacionais. Seguido da exposição e contextualização das realidades presenciadas na Unidade Curricular Estágio Final e Relatório e das suas especificidades, o mesmo prosseguiu segundo as competências específicas do enfermeiro Especialista em Enfermagem Médico-cirúrgica, culminando com a temática central: Qualidade e Segurança dos Cuidados de Enfermagem. Assente numa metodologia maioritariamente descritiva e crítico reflexiva, abordaram-se as atividades e reflexões emergidas do contacto com a prática clínica, capazes de sustentar a aquisição de competências de especialista, assentes na garantia da qualidade dos cuidados prestados e na procura do saber-ser, saber-estar e saber-fazer realçando, ainda, a análise das atividades desenvolvidas, nomeadamente no âmbito da cultura de segurança - onde a revisão bibliográfica culminou com o desenvolvimento de um poster, bem como a identificação de prioridades e necessidades de intervenção na área de especialidade, refletidas no desenvolvimento de um trabalho no âmbito do delirium - com vista a retificar situações problema no cuidado à pessoa em situação crítica na vivência de processos de doença, na resposta a situações de emergência e na intervenção e controlo de infeção e resistência a antimicrobianos. A multiplicidade de atividades e reflexões culminaram num crescimento pessoal e profissional, refletidas na aquisição de competências especializadas bem como o incremento destas na prática clínica diária, com ganhos no trabalho em equipa e na mobilização de conhecimentos que exercitaram e continuarão a retratar uma prática assistencial de enfermagem guiada pela qualidade. Destacam-se assim os contributos ao nível da cultura de segurança, da sua dinamização e desenvolvimento de estratégias de gestão, facilitadoras da manutenção de ambientes terapêuticos seguros, de cuidados uniformizados e de excelência. Importa ainda ressalvar os ganhos na aquisição e consolidação de competências no âmbito da prevenção e controlo de infeção, da mobilização de conhecimentos e na procura de uma prática baseada na evidência mais atual alicerçada numa resposta especializada.
The professional class's commitment to the continuous search and sustainability of nursing knowledge provides an assistance with quality, safety and dignified care, based on scientific science. Integrated in the Master's Course in Nursing with Specialization in Medical-Surgical, in the field of People in Critical Situation, at the Institute of Health Sciences - Porto, Universidade Católica Portuguesa, this report shows the activities and situations of greater relevance in the acquisition and consolidation of skills of Specialist Nurse in the course of the Internship carried out in the context of Urgency and Monovalent Intensive Care Unit. In this sense, this report was organized in such a way as, in a first phase, to make known the context of clinical practice where the Course was developed, A pessoa em Situação crítica e família: vigilância e decisão clínica, aiming to highlight the skills acquired there, namely communicational and relational. Followed by the exhibition and contextualization of the realities witnessed during the Final internship and Report, it continued according to the specific skills of the Specialist Nurse in Medical-Surgical Nursing, ending up in the central theme: Quality and Safety of Nursing Care. Based on a mostly descriptive and reflexive critical methodology, activities and reflections emerged from contact with clinical practice are reported, capable of sustaining the acquisition of specialist skills, based on quality assurance of provided care and the search for know-how, also emphasizing the analysis of activities developed, namely in the context of the safety - where the bibliographic review ended up in the development of a poster, as well as the identification of priorities and the need for intervention in the area of specialty, reflected in the development of a work within the scope of the delirium - with a view to rectifying problems in the peoples care in critical situations experiencing the disease processes, in the response to emergency situations and in the intervention and control of infection and resistance to antimicrobials. The multiplicity of activities and reflections ended up in personal and professional growth, reflected in the acquisition of specialized skills as well as their growth in the daily clinical practice, with gains in teamwork and in the mobilization of knowledge that will continue to portray a guided nursing care practice for quality. The contributions to the level of safety, the dynamism and the development of management strategies that facilitate the maintenance of safe therapeutic environments and excellent care are highlighted. It is also important to highlight the gains in the acquisition and consolidation of skills in the scope of infection prevention and control, the mobilization of knowledge and the search for a practice based on the most current evidence based on a specialized response.
