O presente relatório constitui-se como a representação do percurso realizado na obtenção do grau de mestre em Enfermagem com especialização em Enfermagem Médico-Cirúrgica, pelo Instituto de Ciências da Saúde da Universidade Católica Portuguesa, cujo objetivo principal se centrou na aquisição de competências diferenciadas na prestação de cuidados especializados à pessoa e família a viverem processos de doença relacionados com a ocorrência de queimadura.
Através desta exposição escrita pretendo descrever e analisar reflexivamente as atividades desenvolvidas e as competências adquiridas ao longo dos três módulos que compõem este relatório e que refletem o trabalho realizado durante o período passado no Serviço de Urgência Central (SUC) e na Unidade Multidisciplinar da Dor (UMD) de um Hospital Central de Lisboa, bem como a experiência profissional adquirida ao longo dos 9 anos na Unidade de Queimados da mesma instituição, que me conferiram a creditação do estágio de Cuidados Intensivos.
Selecionei, como eixo condutor para a realização dos estágios e elaboração do presente relatório, o percurso do Doente Vítima de Queimadura desde o momento em que é admitido no Serviço de Urgência, passando pelo tempo de internamento na Unidade de Queimados e culminando com os cuidados após a alta hospitalar. Assim, foi possível compreender a dimensão da problemática da dor crónica no sobrevivente de queimadura no período após alta hospitalar.
O desenvolvimento das competências inerentes ao grau de Enfermeira especialista em Enfermagem Médico-Cirurgica foi realizado tendo em mente uma interação entre a prática clínica, o suporte teórico e as reflexões acerca dos cuidados de Enfermagem e implicou a rutura com uma atuação previamente assumida e o desenvolvimento de práticas alicerçadas no novo saber, promovendo uma desconstrução de pré-conceitos e rotinas e uma posterior construção de novas atuações baseadas na evidência científica. Para tal, a realização de uma Revisão da Literatura norteou o meu processo de pesquisa e consequente crescimento pessoal e profissional, permitindo-me obter respostas à questão “Quais as intervenções farmacológicas e não farmacológicas que promovem o alívio da dor no Sobrevivente de Queimadura e que melhoram a qualidade de vida após a alta?” e desenvolver os diferentes estágios, bem como o presente relatório.
Com base nesta revisão, e na experiência profissional adquirida, defini como objetivo transversal ao estágios contribuir para a melhoria dos cuidados ao Doente Vítima de Queimadura, durante o tempo de permanência no SUC, com a elaboração do póster “O Doente Vítima de Queimadura no SUC” e para a melhoria dos cuidados após a alta, elaborei um projeto de encaminhamento destes doentes para a UMD.
Denote-se que a frequência deste Curso de Especialização em Enfermagem despertou a minha capacidade adaptativa para formas de pensar e atuar diferentes e a criatividade para a resolução de problemas que surgem em contextos de trabalho tão diferentes da minha prática diária, tendo semeando na minha mente a vontade de desenvolver novos projetos na área do saber específica da dor no sobrevivente de queimadura e sua família.
The present report is the representation of the journey in obtaining the master’s degree in Nursing, specialized in surgical and medical nursing, from the Instituto de Ciências da Saúde of Universidade Católica Portuguesa, and the main objective was acquiring differentiated competences for the caring of the person and family living illness processes related with burns. Through this written exposition, I intend to reflectively describe and analyse the developed activities and acquired competences on the three modules that composes this report and reflect the work developed during the training time in the Emergency Service and the Multidisciplinary Pain Unit of a Lisbon Central Hospital, as well as the professional experience acquired during the nine years of work on the Burn Unit of the same institution, which gave me the level of expert and the credits for the Intensive Care Unit module. I’ve selected the path of the Burn Victim since the admission in the Emergency Service, following the time spent in the Burn Unit and ending with the post hospital discharge care as the driving force for the internships and present report. Thus, it was possible to understand the problematic of the chronic pain in burn survivors after hospital discharge. The development of the cited competences was carried out bearing in mind an interaction between clinical practice, theoretical support and reflections about nursing care, and it implied a rupture with a previously assumed performance and the development of practices grounded in new knowledge, promoting a deconstruction of pre-concepts and routines and a later construction of new practices based upon scientific evidence. To do so, conducting a literature review guided my research process and subsequent personal and professional growth, allowing me to obtain answers to the question “Which pharmacological and non-pharmacological interventions promote the pain relief on the burn survivor and improve the quality of life?” develop the internships and the present report. Based upon this review, and the acquired professional experience, I’ve defined as the transversal objective to the internships to contribute for the improvement of care to the Burn Victims, during the time spent in the Emergency Service, with the poster “The Burn Victim in the Emergency Service”, and for the post hospital discharge care, I’ve elaborated a referral project of these patients to the Multidisciplinary Pain Unit. It denotes that to attend this Nursing Specialization Course as awaken my adaptive capacity to different ways of thinking and acting and the creativity to problem solving in contexts so different form may daily practice, having planted in my mind the will to develop new projects in the specific knowledge area of pain in the burn survivor and family.
