Abstract(s)
a reunificação familiar.
The SEGue – Salto Em Grande Project, carried out under the Master's degree in Social Work in the area of Entrepreneurship and Innovation in social economy, of the Portuguese Catholic University, intends to present a proposal for a social project that can respond to young people who are under a foster house measure, as well as support their transition to adulthood, be it autonomy or family reunification. The presented proposal is a digital platform, easily accessible, where young people and technicians can keep in touch and continue, even remotely, supporting and requesting support in the various areas of their lives. This project seeks to minimize the contours of the children’s and young people’s conditions who move to an adult life after a promotion and protection measure, namely that of residential care, building a detailed characterization of the needs of this population. Through the identification of these needs identified, both by young people and professionals, a possible response was developed that can create a transition to a more healing and integrative adult life
