Abstract(s)
Este trabalho constitui um relatório de estágio, que tem como finalidade cumprir formalmente o módulo reflexivo dos contextos de estágio em âmbito de urgência e de cuidados intensivos, como determinado no plano de estudos do Curso de Mestrado em Enfermagem com Especialização em Enfermagem Médico-Cirúrgica: Pessoa em Situação Cítica e Família, da Universidade Católica Portuguesa. Os estágios foram desenvolvidos num serviço de urgência e unidade de cuidados intensivos cardíacos, em Hospitais diferentes, no Distrito de Lisboa. Pretende-se uma exposição de carácter essencialmente crítico-reflexivo sobre as experiências e aprendizagens no âmbito dos estágios, bem como, integrar atividades desenvolvidas na índole de melhoria da qualidade dos cuidados de enfermagem nos contextos de estágio, nomeadamente, um projeto - proposta de organização terapêutica no serviço de urgência pela nomenclatura LASA, concretamente pela organização dos medicamentos do stock, com nome ortográfico e/ou fonético e/ou aspeto semelhante, que podem ser confundidos, originando troca de medicamentos (medicamentos Look-Alike, medicamentos Sound-Alike). Ainda, no âmbito da melhoria da qualidade em contexto clínico, neste relatório é apresentado também um estudo de revisão com o título: “Desenvolvimento de competências dos enfermeiros na abordagem à pessoa em situação crítica através da prática simulada: uma scoping review”, o qual tem como objetivo mapear as competências dos enfermeiros na abordagem aos doentes em situação crítica ao utilizarem a prática simulada. Esta revisão revelou-se determinante no desenvolvimento deste percurso, que tem como core o desenvolvimento de competências à pessoa em situação crítica e respetiva família. Nesta continuidade, a qualidade e a segurança dos cuidados de enfermagem, são o cerne dos fundamentos deste relatório, mobilizando de forma constante competência, qualidade e segurança. Como recurso metodológico foi utilizada a metodologia descritiva, analítica e critico-reflexiva. Os campos de estágio referidos, enquadram contextos clínicos de pessoas em situação crítica, nos quais se preconiza o desenvolvimento dos objetivos de estágio, bem como o desenvolvimento de estratégias e intervenções, que promovam ao longo do estágio, o desenvolvimento de competências comuns e especificas do enfermeiro especialista e que orientem a conduta de intervenção com pensamento intencional, crítico e reflexivo, adequando a vigilância e decisão clínica escrutina análise das situações nas suas conjeturas clínicas, éticas e socioculturais. Projeta-se um resultado transversal do desempenho, nos contextos de estágio, pleno de uma prestação de cuidados de enfermagem especializados, diferenciados, humanizados e fundamentados na evidência científica, a documentar e analisar neste relatório.
This paper aims to fulfill a formality imposed by the curriculum of the Master and the Specialty Degree in Medical-Surgical Nursing: Person in Critical Situation and Family, from the Catholic University of Portugal, in his reflexive module versing the internships in emergency room and intensive care unit. All internships took place on two Lisbon hospitals. This document intents to have a reflexive tone, and a critical one, versing the learning curve, the opportunities the internships provided, as well to include a report of the activities developed in the context of the improvement of nursing care; mainly the elaboration of a proposal of adopting, in the emergency room, the nomenclature LASA in pharmaceutical items. The adoption of his nomenclature is justified by the need to reduce the risk associated with confusing pharmaceutical items: Look-Alike, Sound-Alike. Another initiative developed and presented in this report, aiming to improve nursing care, it’s a scoping review titled: ““critical care nurses’ competencies when using clinical simulation: a scoping review.”, in this aproach the roll-play technic was used as a tool, to identify the skills nurses fall back on dealing with a person in critical care. This review was fundamental in the choices taken in this internship that has, as is main goal, the development of skills in the care of the critical patient and his family. With that guidance, the ‘quality’ and ‘security’ was chosen as the core values of this internship, been use at all steps. As methodological options, the descriptive, analytical and critical-reflexive methodology was used. In the mentioned hospitals, the patients in critical care, are an essential condition to develop the goals of the internship, and implementing the strategies and interventions that make it possible. And doing so, creating the conditions to grow the specific and common skills recognized in a Specialist Nurse, that are a guidance for his interventions supported by critical thought and intent that guides the supervision and clinical decision; assessing the clinical, ethical and socio cultural dimensions. The aim, in this report, is to testify a performance of a pecisalized care, that differs in been differentiated, humanized and based in scientific evidence.
