O presente relatório integra a exposição do percurso que orientou a construção de competências no âmbito do Mestrado em Enfermagem com especialização em Enfermagem Comunitária da Universidade Católica Portuguesa, realizado no Centro de Enfermagem da Católica (CEC). O envelhecimento é um processo caracterizado por perda das capacidades intrínsecas (física e mental) e funcionais da pessoa, exigindo da sociedade em geral, e dos profissionais de enfermagem em particular, intervenções que maximizem a autonomia e independência da pessoa idosa, para um envelhecimento ativo e saudável. De entre as alterações a nível biopsicossocial que marcam esta etapa do ciclo vital, surgem frequentemente modificações na quantidade e qualidade do sono. O sono é um fenómeno de etiologia multifatorial, sensível aos cuidados de enfermagem, que influencia a homeostasia física e mental da pessoa idosa e, consequentemente tem um impacto negativo a nível da funcionalidade dos indivíduos que frequentam os Centros de Dia. Recorrendo à metodologia de Planeamento em Saúde, numa primeira fase, foi efetuado um diagnóstico de situação, num Centro de Dia da cidade do Porto, identificando- se um conjunto de necessidades que, após priorizadas, deram origem ao projeto em estudo, em que foi concebido e testado um procedimento de consulta de enfermagem, com a finalidade de melhorar o sono dos utentes dos Centros de Dia. Na consecução deste projeto, foram elaborados diagramas de árvore de decisão diagnóstica para dar apoio à tomada de decisão, no âmbito da consulta de enfermagem e definiram-se um conjunto de indicadores de estrutura, processo e resultado, que permitem mensurar os efetivos ganhos em saúde, passíveis de serem obtidos com a implementação da consulta. Para finalizar, fez-se uma análise crítico-reflexiva das competências desenvolvidas, no que à Especialidade em Enfermagem Comunitária diz respeito.
This report includes an exposition of the course that guided the construction of competences within the Master's in Nursing with Specialization in Community Nursing at the Universidade Católica Portugesa, held at the Católica Nursing Center. Aging is a process characterized by loss of the intrinsic (physical and mental) and functional capacities of the individual, demanding from society in general, and from the nursing professionals in particular, interventions that maximize the autonomy and the independence of the elderly person, for an active and healthy aging. Among the changes at the biopsychosocial level that mark this stage of the life cycle, there are often changes in the quantity and quality of sleep. Sleep is a phenomenon of multifactorial etiology, sensitive to nursing care, that influences the physical and mental homeostasis of the elderly and, consequently, has a negative impact on the functionality of individuals attending the Day Centers. Based on the methodology of Health Planning, in a first phase, a diagnosis of the situation was carried out in a Day Center in the city of Porto, identifying a set of needs which, after having been prioritized, gave origin to the project under study, in which was conceived and tested a nursing consultation procedure, with the purpose of improving the sleep of the users of the Day Centers. In the context of this project, diagnostic decision tree diagrams were developed to support decision-making in the nursing consultation and a set of structure, process and result indicators were defined to measure the actual gains in health, which can be obtained through the implementation of the consultation. Finally, a critical reflexive analysis of the competences developed was carried out, with respect to the Specialty in Community Nursing.
