O presente relatório é edificado no âmbito do Curso de Mestrado em Enfermagem: Enfermagem Médico-Cirúrgica, na Área de Enfermagem à Pessoa em Situação Crítica da Escola de Enfermagem da Universidade Católica Portuguesa em Lisboa. Tem como objetivo expor o percurso para a aquisição e desenvolvimento de competências com obtenção de grau académico de Mestre em Enfermagem a partir da análise crítico-reflexiva das experiências vividas em contexto clínico. O referencial teórico que orientou o meu pensamento em Enfermagem, norteador do desenvolvimento de competências na prática clínica, integra-se no Paradigma da Transformação e consiste na articulação da Teoria do Cuidado Humano de Jean Watson com a Teoria das Transições de Afaf Meleis. A investigação realizada foi desenvolvida a partir de uma Scoping Review com o tema centrado nas medidas não farmacológicas promotoras de sono para a prevenção de delirium na pessoa em situação crítica. O delirium é caracterizado por um início agudo, com sinais de disfunção cerebral, flutuação e alteração do estado de consciência. A prevenção é a abordagem inicial e caracteriza-se pela utilização de medidas não farmacológicas. O sono é um fator de risco potencialmente modificável com peso na recuperação da pessoa em situação crítica, sendo importante a implementação de intervenções promotoras da sua qualidade. O percurso de aprendizagem foi dividido em dois contextos de estágio: Serviço de Urgência Polivalente e Unidade de Cuidados Intensivos e Intermédios. Transversalmente, foi desenvolvida a investigação que permitiu divulgar os resultados obtidos numa sessão formativa no último contexto de estágio e em evento científico.
This report is constructed within the scope of the Master's Degree in Nursing: MedicalSurgical Nursing, in the Area of Nursing for the Critically Ill from the Portuguese Catholic University’s Nursing School (Lisbon). Its objective is to expose the path towards the acquisition and development of skills with the obtaining of an academic degree of Master in Nursing based on the critical and reflective analysis of experiences lived in a clinical context. The theoretical framework that has guided my thinking in nursing, guiding the development of skills in clinical practice, is part of the Transformation Paradigm and consists of the articulation of Jean Watson's Theory of Human Care with Afaf Meleis' Theory of Transitions. The investigation carried out was developed from a Scoping Review with the theme centered on non-pharmacological sleep-promoting measures for the prevention of delirium in critically ill people. Delirium is characterized by an acute onset, with signs of brain dysfunction, fluctuation and altered state of consciousness. Prevention is an initial approach and is characterized by the use of non-pharmacological measures. Sleep is a modifiable environmental risk factor with an impact on the recovery of the critically ill, and it is important to implement procedures that promote its quality. The learning path was divided into two internship contexts: General Emergency Service and Intensive and Intermediate Care Unit. Transversely, an investigation was developed that made it possible to present the results obtained in a training session at the last internship site and at a scientific event.
