Abstract(s)
O presente relatório surge no âmbito do Curso de Mestrado em Enfermagem Médico Cirúrgica na área de Enfermagem à Pessoa em Situação Crítica, lecionado na Universidade Católica Portuguesa do Porto. Este teve como principal objetivo relatar o percurso realizado no desenvolvimento de competências académicas enquanto mestre e especialista na área em estudo. O interesse em enveredar neste curso de mestrado, surgiu pela necessidade de evolução de capacidades para além dos cuidados generalistas, de forma a direcionar o foco de intervenção para uma área de atuação especializada. Neste sentido, ao longo de todo o documento foi feita uma análise critico-reflexiva acerca do desenvolvimento de competências como enfermeiro especialista, com base em fundamentação teórica e evidência atual. Assim, em termos estruturais começou-se por justificar a escolha desta área de especialidade tendo em conta a experiência anterior. Seguida, do relato do percurso realizado através da apresentação dos objetivos previamente traçados, das atividades desenvolvidas e por fim foram expostas as competências adquiridas nos contextos de estágio. O relatório abrangeu dois períodos de estágio incluídos no âmbito Unidade Curricular “Estágio Final e Relatório”, tendo ambos decorrido em ambiente de cuidados intensivos. O primeiro contexto decorreu num serviço de medicina intensiva polivalente de um centro hospitalar da região do Norte e o segundo num serviço de cuidados intensivos de uma instituição oncológica. O foco principal dos estágios realizados foi o desenvolvimento de idoneidades na abordagem à pessoa em situação crítica. A prática baseada na evidência foi uma das ferramentas presente ao longo deste percurso, pelo que houve a necessidade de realizar uma revisão integrativa da literatura em uma área do saber específica “Ventilação não Invasiva em Pacientes Hemato-oncológicos”. Esta revisão para além de ter potenciado o desenvolvimento de competências como mestre, permitiu o aprimoramento de conhecimentos e melhores práticas no que concerne ao uso de modalidades ventilatórias em doentes particularmente vulneráveis como são os hemato-oncológicos. Concluído este percurso, pode-se considerar que foram desenvolvidas competências inerentes aos domínios comuns e específicos de um enfermeiro especialista, e que este permitiu não só um crescimento pessoal, mas também profissional. Assim, considero-me atualmente um enfermeiro com capacidade de prestar cuidados especializados, com uma visão mais crítica e humanizada, na prestação de cuidados à pessoa em situação critica.
This report is part of a Master's Degree in Nursing with Specialization in Medical-Surgical Nursing in the area of Nursing for Critically ill Patients, taught at the Universidade Católica Portuguesa Porto. The main objective is to report the path taken to obtain the academic competencies as a master and specialist nurse. The interest on embarking this area, was gained for the need to develop capacities beyond generalist nursing care, in order to direct the focus of intervention to a specialized area of action. A critical-reflective analysis was carried out on the development of skills as a specialist nurse, based on the theoretical foundation and current evidence. Structurally, we started by justifying the choice of this area of study taking into account the previous experience. Followed by the description of the route taken, through the presentation of the previously outlined objectives, the activities developed and the skills acquired in the internship contexts. The report covered two placement periods, that took place at an intensive care unit, part of the curricular unit “Estágio Final e Relatório”. The first context was carried out in a general intensive care unit at a North region hospital and the second context took place at an intensive care unit of an oncology institution. The main focus of this placements was the development of skills at caring a critical ill person. Evidence-based practice was one of the tools that was always present along this path, so there was a need to carry out an integrative literature review in a specific area of knowledge “Non-invasive ventilation in hematologic malignant patients”. This review, in addition to enhancing the development of skills as a master, allowed the improvement of knowledge and best practices with regard to the use of ventilatory modalities in patients with hematologic malignancies. Completed this course, it can be considered that skills inherent to the common and specific domains of a specialist nurse were developed, and that this allowed not only individual but also professional growth. Thus, I consider myself a nurse with the ability to provide specialized care, based on a critical and humanized view at providing care to critically ill patients.
