O presente relatório pretende demonstrar o percurso na aquisição e desenvolvimento de competências clínicas especializadas no âmbito do Mestrado em Enfermagem Médico Cirúrgica na Área de Especialização em Enfermagem à Pessoa em Situação Crítica, do Instituto de Ciências da Saúde - Lisboa, da Universidade Católica Portuguesa. A teorização de enfermagem com recurso à Teoria da Incerteza de Merle Mischel, foi integrada como guia do pensamento em Enfermagem no desenvolvimento do cuidado de excelência. No decurso do meu percurso clínico, dinamizei os resultados de investigação do cuidado, abordando o fenômeno da melhoria contínua da qualidade e segurança da pessoa em situação crítica durante o transporte intra-hospitalar, identificando fatores que propiciem a ocorrência de eventos adversos, de modo a implementar medidas que antecipem e corrijam estes. Deste modo, foi realizada uma scoping review, com o intuito de mapear a literatura, com base no modelo preconizado pelo Joanna Briggs Institute para extração de dados. A segurança é um elemento fundamental para a qualidade dos cuidados e saúde em qualquer parte do mundo e, é considera uma prioridade. Para garantir a segurança no cuidado é importante implementar intervenções que evitem, previnam e melhorem a ocorrência de eventos adversos que possam prejudicar a saúde da pessoa em diversas dimensões. Estes eventos adversos podem ser irreversíveis, acarretar custos socioeconômicos e aumentar a morbilidade e mortalidade da pessoa. Situações complexas como o transporte de pessoas em situação crítica, há uma tendência, maior, para a ocorrência de eventos adversos que, se devidamente previstos, podem ser evitados e/ou corrigidos. Assim, torna-se crucial a adoção de medidas preventivas e corretivas para garantir a segurança das pessoas submetidas ao transporte e profissionais envolvidos no mesmo. De modo a integrar o saber teórico no cuidado especializado, encontra-se, no presente relatório, a análise fundamentada e reflexão crítica do processo de aquisição, desenvolvimento e aprofundamento de competências comuns e específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica, a Pessoa em Situação Crítica e as necessárias à obtenção do grau de Mestre em Enfermagem desenvolvidas nos dois contextos clínicos: Unidade de Cuidados Intensivos e na área da emergência pré-hospitalar.
This report aims to demonstrate the path towards acquiring and developing specialized clinical skills within the scope of the Master's Degree in Medical-Surgical Nursing, in the specialization area of Person in Critical Condition, at the Institute of Health Sciences - Lisbon, of the Portuguese Catholic University. Nursing theorization using Merle Mischel's Uncertainty Theory was integrated as a guide to Nursing thinking in the development of excellent care. Throughout my clinical journey, I promoted the research results of care, addressing the phenomenon of continuous improvement in the quality and safety of the person in critical condition during intra-hospital transport, identifying factors that contribute to adverse events, in order to implement measures that anticipate and correct them. Thus, a scoping review was conducted to map the literature, based on the model recommended by the Joanna Briggs Institute for data extraction. Safety is a fundamental element for the quality of care and health anywhere in the world and is considered a priority. To ensure safety in care, it is important to implement interventions that avoid, prevent, and improve the occurrence of adverse events that may harm the person's health in various dimensions. These adverse events can be irreversible, entail socio-economic costs, and increase the morbidity and mortality of the person. In complex situations such as the transport of people in critical condition, there is a greater tendency for adverse events to occur that, if properly anticipated, can be avoided and/or corrected. Thus, the adoption of preventive and corrective measures is crucial to ensure the safety of people undergoing transport and professionals involved in it. In order to integrate theoretical knowledge into specialized care, this report presents a grounded analysis and critical reflection of the process of acquiring, developing, and deepening common and specific competencies of the Nurse Specialist in Medical-Surgical Nursing, the Person in Critical Condition, and those necessary for obtaining a Master's degree in Nursing developed in two clinical contexts: Intensive Care Unit and in the pre-hospital emergency area.
