Resumo(s)
Este Relatório plasma o desenvolvimento do Estágio na área de especialização em
Enfermagem Comunitária e as competências adquiridas neste percurso. Contemplou três
módulos (I, II e III) que decorreram nos períodos entre 20 de Abril e 20 de Junho de
2009, 6 de Outubro e 27 de Novembro de 2009 e 2 de Dezembro e 31 de Janeiro de
2010, respectivamente, no Centro de Saúde Vila Real I.
Na sequência do Módulo I, elaborou-se um diagnóstico de saúde relativo à
Adesão/Gestão do Regime Terapêutico e Autovigilância na Diabetes. Delinearam-se
prioridades, surgindo como área de intervenção a autovigilância do pé nas pessoas com
diabetes tipo 2. No segundo módulo, foi desenhado um projecto de intervenção (Pés +
Cuidados: Projecto Educacional para Pessoas com Diabetes Tipo 2, que procurou a
capacitação do utente diabético para o desenvolvimento de habilidades e competências
ao nível da autovigilância do pé), executado e avaliado e implementada a I Feira da
Diabetes.
Relativamente ao Módulo III, procurei perceber de que modo a Equipa de Cuidados
Continuados Integrados se articula com as restantes unidades funcionais, a organização e
dinâmicas da equipa ao nível da gestão e prestação de cuidados. Face aos problemas
percepcionados pela Equipa, foi realizado um Workshop subordinado ao tema: “ECCI, que
desafios?”.
Concluindo, este Estágio foi rico do ponto de vista da diversidade de experiências que
proporcionou a aquisição de competências especializadas em Enfermagem Comunitária.

Included three modules (I, II and III) that took place in between April 20 and June 20, 2009, 6 October and 27 November 2009 and December 2 to January 31, 2010, respectively at Vila Real I Health Care Center. Following the module I was elaborated a diagnosis of health on Adherence/ Management of the therapeutic regimen and the Diabetes Self-monitoring. Were outlined priorities, emerging as a target area the foot self-monitoring in people with type 2 diabetes. In the second module, was designed an intervention project (+ Foot Care: Educational Project for People with Diabetes Type 2, which sought to train the diabetic people to develop skills and competencies at the level of self-monitoring of foot), executed and evaluated and was implemented the First Diabetes Fair. With regard to Module III, tried to see how the ECCI unit was articulated with other functional units, the organization and dynamics of the team in the management and care. Given the problems perceived by the Team was held a workshop entitled: "ECCI, which challenges?". In conclusion, this stage was rich in terms of the diversity of experiences that provided skills and expertise in community nursing.
