A sociedade depara-se com desafios constantes, verificando-se um crescente envelhecimento populacional, pelo que o tema dos cuidados à população idosa constitui uma preocupação central no cuidar em enfermagem, por ser um grupo de risco para doenças cérebro cardiovasculares, que constituem a principal causa de morte, de morbilidade, de incapacidade e invalidez e de anos potenciais de vida precocemente perdidos. Reitera-se uma intervenção estruturada, pelo que é indispensável a atuação do enfermeiro em contexto comunitário, de forma a fazer emergir o potencial máximo de saúde e bem-estar das pessoas idosas. A literacia em saúde assume-se de forma inigualável como um poderoso propósito para promover a sua saúde. O presente relatório espelha o projeto de intervenção comunitária: “Literacia em saúde: capacitar a comunidade sénior para a prevenção das doenças cérebro cardiovasculares”, concretizado com um grupo de 9 seniores de uma academia sénior, no âmbito do estágio na Unidade de Saúde Pública Arnaldo Sampaio. A elaboração deste projeto teve por base a metodologia de planeamento em saúde proposta por Imperatori e Giraldes e foi suportado pelo modelo de promoção de saúde de Nola Pender. Para a execução do diagnóstico de situação realizou-se um brainstorming sobre as doenças cérebro cardiovasculares com a população-alvo. Com base nos resultados obtidos efetuou-se a priorização dos problemas e definiu-se como prioritária a intervenção no diagnóstico de enfermagem conhecimento comprometido do grupo sobre doenças cérebro cardiovasculares. Foi definido como objetivo geral do projeto: Contribuir para a capacitação de um grupo de seniores da academia sénior X para a prevenção de doenças cérebro cardiovasculares, de 9 de setembro a 19 de dezembro de 2024. Foram definidos objetivos específicos, metas e indicadores, e fixadas estratégias. Foi utilizada a estratégia educacional e estabelecidas parcerias na comunidade, tendo sido realizadas sessões de educação para a saúde, a elaboração de um e-book com a população-alvo e a divulgação do projeto junto de outros seniores, promovendo a sua continuidade. Foram alcançadas as metas estabelecidas. Este percurso permitiu o desenvolvimento de competências de Mestre e de Enfermeiro Especialista em Enfermagem Comunitária e de Saúde Pública.
Society faces constant challenges, with a growing aging population, so the issue of care for the elderly is a central concern in nursing care, being a risk group for cardiovascular diseases, which are the leading cause of death, morbidity, disability, and years of potential life lost prematurely. A structured intervention is reiterated, so the role of the nurse in a community context is indispensable, in order to bring out the maximum health and well being potential of elderly people. Health literacy is uniquely positioned as a powerful purpose to promote their health. The present report reflects the community intervention project: "Health Literacy: Empowering the Senior Community for the Prevention of Cardiovascular Brain Diseases", implemented with a group of 9 seniors from a senior academy, as part of an internship at the Arnaldo Sampaio Public Health Unit. The preparation of this project was based on the health planning methodology proposed by Imperatori and Giraldes and was supported by Nola Pender's health promotion model. To execute the situational diagnosis, a brainstorming session was held about cardiovascular brain diseases with the target population. Based on the results obtained, the problems were prioritized, and it was defined that the intervention in nursing diagnosis regarding the group's compromised knowledge of cardiovascular brain diseases was a priority. The general objective of the project was defined as: Contributing to the training of a group of seniors from the senior academy X for the prevention of cerebrovascular and cardiovascular diseases, from September 9 to December 19, 2024. Specific objectives, goals, and indicators were defined, and strategies were established. An educational strategy was used and partnerships in the community were formed, with health education sessions held, an e book created with the target population, and the project promoted among other seniors, ensuring its continuity. The established goals were achieved. This journey allowed for the development of skills as a Master and Specialist Nurse in Community and Public Health Nursing.
