O presente relatório assume-se como um retrato do caminho para a aquisição de competências no Mestrado em Enfermagem Médico-Cirúrgica, na área da Enfermagem à Pessoa em Situação Crítica, durante a realização dos estágios no Serviço de Nefrologia e no Serviço de Medicina Intensiva de dois Hospitais da zona Norte de Portugal. Este documento foi elaborado tomando como referencial as Competências Comuns do Enfermeiro Especialista, representado um reporte formal do desenvolvimento de atividades e experiências que permitiram a aquisição de competências na área de especialização da Enfermagem à Pessoa em Situação Crítica. Do caminho percorrido para a mestria e desenvolvimento de competências especializadas no cuidado à pessoa em situação crítica, são descritas as principais atividades desenvolvidas, destacando-se neste relatório: a construção de um referencial teórico relativamente à Best Practice na colheita de hemoculturas à pessoa com suspeita de infeção da corrente sanguínea associada ao Cateter Venoso Central, que culminou na atualização de uma instrução de trabalho para a colheita de hemoculturas para o Serviço de Nefrologia; a criação do Projeto ID-SMI, projeto de investigação piloto que visa estabelecer sinergias, numa abordagem colaborativa, entre a Universidade Católica Portuguesa e um contexto da prática de cuidados (Serviço de Medicina Intensiva); e a adaptação e validação de um instrumento de colheita de dados que permite a avaliação do conhecimento dos Enfermeiros relativamente ao Feixe de Intervenções para Prevenção da Infeção associada ao Cateter Vascular Central. Destacam-se, neste relatório, as aprendizagens no âmbito do cuidado à pessoa em situação crítica e família/cuidador, da promoção da humanização dos cuidados, dos processos de prevenção e controlo da infeção e de resistência aos antimicrobianos, e do desenvolvimento dos cuidados à luz das conceções da prática baseada em evidência, com destaque para a diferenciação destas temáticas no scope de intervenção do Enfermeiro Especialista.
This report is a portrait of the path towards the acquisition of skills in the Master's Degree in Medical-Surgical Nursing, in the field of Nursing for the Person in Critical Condition, during the internships in the Nephrology and Intensive Care Medicine Departments of two Hospitals in the North of Portugal. This document was constructed taking the “Common Skills of the Specialist Nurse” as a reference, representing a formal report on the fulfillment of activities and experiences that allowed the acquisition of skills in the area of specialization of Nursing for the Person in Critical Condition. From the path taken towards the expertise and development of specialized skills in critical care, the main activities carried out are described, such as: the construction of a theoretical framework regarding the Best Practice in the blood culture collection from people with suspected infection of the bloodstream associated with central lines, which culminated in the update of a guideline for blood culture withdrawal in the Nephrology Department; the creation of the ID-SMI Project, a pilot research project which aims to establish synergies, in a collaborative approach between Universidade Católica Portuguesa and the clinical care practice (Intensive Care Unit); and the adaptation and validation of a data collection tool that allows for the assessment of Nurses' knowledge regarding the Bundle of Interventions for the Prevention of Infection associated with the Central Vascular Catheters. In this report, learnings are highlighted regarding the care for critically hill patients and family/caregiver, promoting humanized care, processes of prevention and control of infection and resistance to antimicrobials, and the development of care carried under evidence-based practice conceptions, highlighting the differentiation of these themes in the scope of the intervention of the Specialist Nurse.
