Introdução: A prestação de cuidados paliativos (CP) e o alívio do sofrimento são considerados uma questão ética, de saúde pública e respeito pelos direitos humanos. A especialidade de Medicina Geral e Familiar (MGF) encontra-se numa posição privilegiada na prestação de CP generalistas, permitindo uma maior cobertura e acessibilidade a estes cuidados. Contudo, a formação dos profissionais tem sido apontada como o principal obstáculo pelo que é fundamental definir e dotar os profissionais de competências básicas. Assim, a presente dissertação tem como objetivo listar essas mesmas competências nos domínios da dor e controlo de sintomas e questões éticas e legais. Métodos: Scoping review segundo a metodologia do Joanna Briggs Institute (JBI), cuja pesquisa foi efetuada em dezembro de 2019. Foram incluídos estudos publicados em base de dados internacionais e literatura cinzenta nas línguas inglesa, espanhola, francesa e portuguesa, sem limite temporal e com uma descrição objetiva e clara das competências básicas em CP para a especialidade de MGF nos domínios da dor e controlo de sintomas e questões éticas e legais. Resultados: Foram obtidos 4958 registos em análise de base de dados internacionais e 15 de literatura cinzenta. Destes foram incluídos 4 artigos de base de dados internacionais e 7 artigos de literatura cinzenta resultando na identificação de 135 competências básicas em MGF nos domínios da dor e controlo de sintomas (89) e questões éticas e legais (46). Conclusão: A MGF tem proporcionado CP aos seus doentes e família na sua prática clínica diária. Contudo, até à data, poucos dados existiam acerca das competências básicas necessárias para a prestação de CP de qualidade nos domínios da dor e controlo de sintomas e questões éticas e legais para a especialidade em Portugal. Com os dados recolhidos nesta scoping review esperamos colocar este tema em discussão, permitindo a elaboração de um currículo formal para a especialidade.
Introduction: The provision of palliative care (PC) and the alleviation of suffering are considered an ethical, public health and respect for human rights issue. The general practice/family medicine specialty is in a privileged position in the provision of general CP, allowing greater coverage and accessibility to this care. However, the training of professionals has been identified as the main obstacle, so it is essential to define and provide professionals with basic skills. Thus, the present dissertation aims to list these same competences in the domains of pain and symptom control and ethical and legal issues. Methods: Scoping review according to the JBI methodology, whose research was carried out in December 2019. Studies published in international databases and gray literature in English, Spanish, French and Portuguese were included, without time limit and with an objective and clear knowledge of basic PC skills for the general practice/family medicine specialty in the domains of pain and symptom control and ethical and legal issues. Results: 4958 records in analysis from international databases and 15 from gray literature. Of these, 4 international database articles and 7 gray literature articles were included in the review resulting in the identification of 135 basic competencies in general practice/family medicine specialty in the domains of pain and symptom control (89) and ethical and legal issues (46). Conclusion: General practice/family medicine specialty has provided PC to its patients and families in their daily clinical practice. However, to date, little data exists on the basic competencies needed to provide quality PC in the domains of pain and symptom control and ethical and legal issues for the specialty, in Portugal. Thus, with the data collected in this scoping review, we hope to put this topic under discussion, allowing the development of a formal curriculum for the specialty.
