  • Dental implant outcomes in grafted sockets: a systematic review and meta-analysis

    Objectives: To assess the treatment outcomes of the dental implants placed in the grafted sockets.Material and Methods: A search protocol was developed to evaluate the treatment outcomes of dental implants placed in the grafted sockets in terms of implant survival rates (primary outcome), marginal-bone-level (MBL) changes, clinical parameters (i.e., bleeding on probing, probing depth), occurrence of peri-implant diseases, and aesthetic outcomes (secondary outcomes). Randomized controlled clinical trials (RCTs), controlled clinical trials, and prospective studies with at least 12 months of follow-up and a minimum of 10 patients having at least one dental implant inserted into the grafted socket were conducted. MEDLINE (PubMed) was searched for relevant articles published until 1st April 2019. A meta-analysis was performed using the random-effects model on the selected qualifying articles.Results: The present analysis included 7 RCTs. The survival rate of the implants inserted into the grafted sockets ranged from 95 to 100% after 1 to 4 years of follow-up. MBL loss was found to be significantly greater for the implants placed in the non-grafted healed sites than for those placed in the previously grafted sockets (weighted mean difference = -1.961 mm, P < 0.0001). In terms of MBL changes, no difference was detected between immediately inserted implants versus implants placed in previously grafted sockets. None of the included studies reported on the clinical parameters or occurrence of peri-implant diseases.Conclusions: Implants inserted into the previously grafted sockets showed high survival rates and lower marginal-bone-level loss than the implants inserted into the non-grafted sites.
  • Biocompatibility and effectiveness of a novel, organic olive oil-based denture adhesive: a multicenter randomized and placebo-controlled clinical trial

    To assess the clinical efficacy of a novel, organic olive oil-based denture adhesive and its effect on Candida albicans growth in maxillary edentulous individuals wearing complete dentures, individuals were selected from two dental schools in Portugal and Spain. Twenty-eight complete dentures were relined, following a standardized protocol. The novel product (test) was compared with a commercialized adhesive (control) and Vaseline (placebo) randomly assigned in a cross-study design. The retention resistance was measured with a gnathometer and a dynamometer. The patients related outcome evaluations with a five-point questionnaire, and the Candida albicans growth in a Sabouraud dextrose agar (SDA) medium was used to evaluate differences between the placebo and experimental product. Twenty-three participants were included. The dynamometer evaluation showed significant differences between not using a denture adhesive and using either (experimental, p = 0.03; control, p = 0.04) and no significant differences between the two adhesives (p > 0.05). In the subjective analysis, the experimental adhesive showed a significantly longer effectiveness (p = 0.001), and the control reported better results in taste (p = 0.03) and in chewing (p = 0.001). The test adhesive showed better (p < 0.001) Candida albicans growth inhibition. The experimental adhesive showed longer effectiveness than the control and the placebo with a better inhibition capacity for the growth of Candida albicans. Patients reported better abilities for speech, chewing, taste, and retirement in the control adhesive.
  • Oral health behaviors and dental caries in a sample of Portuguese militaries

    Objectives: To determine the prevalence of dental caries and oral health behaviors in a sample of the Portuguese army. Methods: An observational cross-sectional study was conducted in a sample of army soldiers of the Infantry Regiment no. 14 of Viseu, Portugal, using a questionnaire. The study involved 122 members of the armed forces, who were asked to complete a questionnaire autonomously. The questionnaire enquired about general oral-health behaviors but focused mainly on: i) frequency of toothbrushing, ii) use of dental floss and iii) frequency of dental appointments. It also asked about sociodemographic information, like age and gender. An intra-oral observation was also conducted to determine the DMFT index and to record dental plaque based on the Silness and Löe plaque index (1964). Results: The prevalence of dental caries was high (51.6%±7.4%; DMFT of 4.6±3.5). The most prevalent score of the Silness and Löe plaque index was score 2 - visible plaque (58.2%). Regarding oral hygiene habits, most of the sample reported brushing their teeth at least twice a day (65.6%) and using fluoride toothpaste (64.8%). However, very few participants (18%) used dental floss. More than 30% of the participants had not visited a dentist within the previous year. Furthermore, 45.9% were current smokers. Conclusion: Prevention programs and promoting actions for oral health with these specific groups are important and should be adopted to reduce the prevalence of dental caries and to increases the knowledge about oral-health behaviors.

  • Effect of bone loss on the fracture resistance of narrow dental implants after implantoplasty. An in vitro study

    Background: Implantoplasty (IP) involves polishing of the exposed surface of implants affected by peri-implantitis (PI). A study was made to determine whether the degree of bone loss influences the fracture resistance of implants with or without IP. Material and Methods: An in vitro study was carried out on 32 narrow (3.5 mm) dental implants with a rough surface and external hexagonal connection. Implantoplasty was performed in half of the implants of the sample. Both the IP and control implants were divided into two subgroups according to the amount of bone loss (3 mm or 7.5 mm). Standardized radiographic assessment of implant width was performed using specific software. The main outcome variable was the maximum compression force (F-max) of implants when subjected to static resistance to fracture tests. Implant fractures were subsequently analyzed by scanning electron microscopy. A descriptive and bivariate analysis of the data was performed. Results: Significant changes in implant width were observed after IP (p<0.05). No significant differences between IP and control implants were recorded in terms of the F-max values in the two bone loss subgroups (3 mm: control 854.37N +/- 195.08 vs. IP 752.12N +/- 186.13;p =0.302, and 7.5 mm: control 548.82N +/- 80.02 vs. IP 593.69N +/- 111.07; p-0.370). Greater bone loss was associated to a decrease in F-max which proved significant for the control implants (p=0.001). Fractures were more frequently located in the platform (n=13). Conclusions: Implants with more apical bone levels appear to be more susceptible to fracture. On the other hand, IP does not seem to significantly decrease the fracture resistance of narrow (3.5 mm) platform dental implants with external hexagonal connections. The fact that most fractures occur in the platform area indicates that the latter is exposed to more mechanical stress.
  • Utilização das tecnologias de informação e comunicação numa faculdade de medicina dentária portuguesa

    Objetivos: As tecnologias de informação e comunicação (TIC) têm um papel crucial na divulgação e na partilha de conhecimento, tanto para profissionais como estudantes. O objetivo deste estudo visa avaliar as habilitações de docentes e discentes de medicina dentária em algumas áreas específicas das TIC. Métodos: A metodologia consistiu num estudo observacional transversal (técnica de amostragem estratificada), efetuado aos docentes do ciclo clínico e discentes do Mestrado Integrado em Medicina Dentária da Universidade Católica Portuguesa, aos quais foi distribuído um questionário para avaliação da utilização das TIC. Resultados: Os resultados obtidos indicam competência «Mais do que adequada» (NC = 33,3) no manuseamento das TIC, sendo que a maioria dos inquiridos (66%) afirmou ter recebido formação «Adequada» em TIC no decorrer do seu mestrado integrado/licenciatura. Entre outros factos, verificou-se que, quanto à pesquisa on-line de informação médica, a maioria (54,4%) dos inquiridos afirma saber utilizar a Medline®, mas gostaria de aprender mais. Conclusões: A amostra e o centro de ensino estudados apresentam condições adequadas à implementação de TIC no ensino de medicina dentária. As TIC são consideradas essenciais no futuro do ensino e prática clínica de medicina dentária.
