- Health-illness transition processes in children with complex chronic conditions and their parents: a scoping reviewPublication . Loura, David; Ferreira, Ana Margarida; Romeiro, Joana; Charepe, ZaidaBackground: The prevalence of complex chronic conditions (CCC), which cause serious limitations and require specialized care, is increasing. The diagnosis of a CCC is a health-illness transition for children and their parents, representing a long-term change leading to greater vulnerability. Knowing the characteristics of these transitional processes is important for promoting safe transitions in this population. This scoping review aimed to map the available evidence on health-illness transition processes in children with complex chronic conditions and their parents in the context of healthcare. Methods: Six databases were searched for studies focusing on children aged 0–21 years with CCC and their parents experiencing health-illness transition processes, particularly concerning adaptation to illness and continuity of care, in the context of healthcare. Studies within this scope carried out between 2013 and 2023 and written in Portuguese or English were identified. The articles were selected using the PRISMA methodology. The data were extracted to an instrument and then presented with a synthesizing approach supporting the interpretation of the results. Results: Ninety-eight methodologically broad but predominantly qualitative articles were included in this review. Children with CCC have specific needs associated with complex and dynamic health-illness transitions with a multiple influence in their daily lives. Several facilitating factors (p.e. positive communication and a supportive therapeutic relationship with parents and professionals, as well as involvement in a collaborative approach to care), inhibiting factors (p.e. the complexity of the disease and therapeutic regime, as well as the inefficient organization and coordination of teams) and both positive (p.e. well-being and better quality of life) and negative response patterns (p.e. negative feelings about the chronic illness) were identified. Some interventions to support the transitional process also emerged from the literature. Pediatric palliative care is seen as a good practice and an integrative approach for these children and families. Conclusion: Health professionals play a fundamental role in supporting the transitional process and promoting positive response patterns. More significant investment is needed at the clinical and academic levels regarding production and dissemination of knowledge in this area to ensure the awareness of children with CCC and that their needs are fully enhanced.
- Hopelessness a predictor of spiritual distress nursing diagnosis in cancer patients: a follow-up studyPublication . Martins, Helga; Romeiro, Joana; Caldeira, SílviaBackground Spiritual distress is defned as a lack of meaning in life, disconnection, sufering and anger at God. Also, spiritual distress, is a nursing diagnosis according to NANDA-International, Inc, which hopelessness is one of the defning characteristics. The aim of this study was to assess hopelessness as a predictor of in spiritual distress nursing diagnosis in cancer patients during chemotherapy treatment. Materials and methods A quantitative, observational and follow up study. It was comprised 322 outpatients with cancer followed at an Oncology Unit, randomly selected. The data was collected when patients were initiating chemotherapy and then quarterly until completing one year of chemotherapy. In addition, data collection occurred between February 2019 and May 2015, and was applied a questionnaire. The data was analyzed statistically through the program SPSS version 21. This study, was approved by the hospital´s Ethics Committee where the study was conducted. Results The initial sample embraced 188 females and 144 males. The dropout rate of this study reached 17,5% after twelve months. The age range was between 22 to 83 years old. Most of the patients had breast cancer (n=27.70%), colorectal cancer (n=23.7%) and lung cancer (n=14.8%). The frequency of hopelessness during the chemotherapy increased its value, such as before chemotherapy (n=55, 10.5%), after three months (n= 40, 12.7%), after six months (n= 41, 13.5%), after nine months (n= 40, 14.0%) and after twelve months (n= 42, 15.3%). Hopelessness was a predictor of spiritual distress after nine (ꞵ=3.254, p=0.029) and twelve months (ꞵ=3.461, p=0.019) since the beginning of chemotherapy. Conclusions At the end of nine months after the start of chemotherapy, hopelessness appears as a predictive variable for the diagnosis of spiritual distress, therefore nurses can anticipate autonomous nursing interventions with the purpose for the readiness for enhanced hope in cancer patients.

- Nursing teaching strategies used to promote collaboration and teamwork: an integrative literature reviewPublication . Romeiro, Joana; Figueiredo, Amélia SimõesObjective To identify in nursing literature published between 2010-2014 the teaching strategies capable of promoting collaboration and teamwork. Methods This integrative literature review allowed the selection of empirical studies in nursing literature published between January 2010 and December 2014. The review was taken in April 2015, using the EBSCOhost platform to search in databases such as CINAHL® Plus with Full Text, Nursing & Allied Health Collection, Cochrane Collection, MedicLatina and MEDLINE with Full Text, Academic Search, and Education Research Complete. Results Findings revealed 18 empirical studies. Academic approaches based on high and low-fidelity simulations, “role play”, work group, reflective discussions and clinical stages were the strategies employed. Conclusions The present study emphasizes the need to develop instruments that can be used in the evaluation of the competence to collaborate and work as a team in students in the first years of nursing studies.
- PO04 – Metasynthesis approach in paediatric nursing literature: a scoping reviewPublication . Romeiro, Joana; Caldeira, SílviaIntroduction: Metasynthesis is defined as the synthesis of qualitative research. Despite the growing interest in this approach in adult nursing literature, it is not clear how it has been used in paediatric nursing literature. Aims: To characterize the scientific production using metasynthesis in paediatric nursing literature. Methods: A scoping review was conducted across eight international databases and three Portuguese nursing web journals. Results: A total of 25 papers were included. First was published in 1998, most frequents phenomena were parenting and experiences of parents in several health contexts. Metasynthesis (n=10) and meta-meta-ethnography (n=8) were most frequent. Noblit and Hare (1988) framework was most cited and sample ranged from 3 to 47 original studies. Discussion: Metasynthesis research is providing evidence to nursing practice mostly in neonatal and intensive care regarding parenting. Conclusions: Metasynthesis is being use in paediatric nursing, but in specific contexts and phenomena. Other qualitative research should be analyzed toward metasynthesis.
- Portuguese version of the spiritual well-being questionnaire: validation study in people under assisted reproductive techniquesPublication . Romeiro, Joana; Nogueira, Paulo Jorge; Fisher, John; Caldeira, SílviaAttention has been given to the experience of individuals undergoing assisted reproductive techniques. However, only recent literature has focused on the spiritual journey triggered by such an event and highlighted the nurses and midwives’ roles in the assessment of the spiritual needs of those living with infertility. This study aimed to perform a psychometric test of the factor structure of the Portuguese version of the Spiritual Well-being Questionnaire in a sample of people undergoing assisted reproductive techniques. This descriptive cross-sectional study was conducted between September 2019 and June 2020 on a sample of 104 Portuguese adults attending fertility treatment. An online questionnaire was released on fertility-related websites. A confirmatory factor analysis was conducted on six hypothesized models of the instrument. The findings provided evidence of an adequate internal consistency of the instrument, and the validity and reliability of the oblique four-factor model was confirmed. Spiritual well-being had a significant association with the nature of infertility and a higher score was achieved by participants that perceived religion as very important. This study provides a validated tool for nurses and midwives to assess spiritual well-being and promote an integrated reproductive healthcare approach. Further longitudinal research with bigger samples would provide more evidence of the spiritual needs of people living with this condition.
