- Edentulism and the need of oral rehabilitation among institutionalized elderlyPublication . Despite advances in preventive dentistry, edentulism is still a major public health issue in Portugal. Objectives: Determine the prevalence of edentulism and assess the oral rehabilitation and oral/prosthetic hygiene habits in a sample of institutionalized elderly. Methods: We conducted a cross-sectional study in a sample of 68 institutionalized elderly (79.4 % female), in which the average age was 78.3 ± 12.0 years old. Data collection was performed by applying a questionnaire about oral health behaviours and through an intraoral observation in order to determine the oral status and verify the condition of the oral rehabilitation of each participant. Results: In the present study, 58.8 % presented total edentulism with no natural teeth in the oral cavity and 66.7 % had a removable prosthesis. From the elderly that had a removable prosthesis, only 42.9 % presented satisfactory prosthetic retention and stability. Only 44.1 % refer daily oral/prosthetic hygiene at least twice-a-day. From the total sample, 29.0 % refer having toothache, 58.1 % refer dry mouth and 67.7 % refer difficulty in chewing, even in the case of having a removable prosthesis. The educational level of the elderly was associated with dental pain (p = 0.012) and oral/prosthetic hygiene (p = 0.034). Conclusions: A high prevalence of tooth loss was found in the sample studied and also the need of improvement of the removable prosthesis is fundamental. Improvement in oral healthcare and oral hygiene habits is essential to promote better oral health and quality of life among the institutionalized elderly.
- Oral health and physical and mental limitations among the elderlyPublication . During this phase pathophysiological changes appear that may affect the whole body, and, more specifically, the oral cavity. The physical and cognitive limitations are those that most affect the oral health, especially among the elderly. This study aims to conduct a literature review about oral health among the elderly with physical and mental limitations, in particular with Alzheimer’s disease, Parkinson’s disease and rheumatoid arthritis. To carry out this review article the search strategies included electronic databases, reference lists of articles, and selected textbooks. Articles and textbooks used in this study were mainly reached by using the following keywords: “oral health”, “elderly”, and “stroke”, “Parkinson’s disease”, “Alzheimer’s disease”, “musculoskeletal disorders. By the end of the research, 16 scientific articles were selected. The prevalence of oral health diseases increase with age and is highly associated with neurological disease development. Therefore, the maintenance of the quality of life among the elderly should have in consideration the risk of neurological disease development and the influence that theses outcomes may have in the oral health of the elderly. Dental health professionals must be highly aware of the limitations that these patients present in undergoing their own daily oral hygiene and tha higher risk of oral disease development.
- Oral lesions associated with removable prosthesis among elderly patient'sPublication . Removable dentures purpose is mainly the oral rehabilitation of edentulous areas. Some secondary purposes for the dental prosthesis include elimination of disease, restoration of the remaining teeth and preservation of the surrounding oral tissues. Due to life expectancy rose, removable dentures wear is frequent nowadays. However, their functional and qualitative limitations can lead to oral lesions. The aim of this study is to present several oral lesions caused by dentures wear and their clinical characteristics among elderly patients. It is based on a literature review, intending to emphasize the importance of proper control and denture adaptation to oral environment. A good manufacturing and hygiene of a denture is of great relevance in order not to compromise the oral mucosa. Removable dentures won’t replace a patient’s normal dentition. Their repeated use can cause denture lesions, leading to mastication inefficiency and patient’s decreasing nutritional capacity. These lesions are more common when there is a poor oral hygiene. Thus, the health professional´s instructions are of great importance as well as removable dentures and patient’s oral health monitoring and periodic assessments.
- Review on polypharmacy and oral health among the elderlyPublication . This fact can be explained by the improvement of living conditions of the populations, advances in medicine, improved sanitation and increase in the community and individual health care. The use of several drugs, self-medication, drug side effects, several drug interactions and decreased quality of life of the population has become an important public health issue. The gradual aging of the population and the increasing prevalence of chronic diseases has led to polypharmacy and the use of drugs that potentially are considered inappropriate for using among the elderly population. Oral diseases, such as periodontal disease, dental caries, lesions in the oral mucosa and xerostomia could be complications associated with polypharmacy among the elderly. It is crucial that the health professional, using his contemporary knowledge, is able to protect the elderly population from complications caused by polypharmacy. The purpose of this article was to highlight the importance of knowledge and recognition of the risks that polypharmacy may represent during dental treatment based on the international scientific literature.

The most affected systems in the elderly are the neurological, cardiovascular, respiratory, endocrine and muscularskeletel. Overall, all they affect a special system: the oral cavity. The objective of this review article consists in characterizing the influence of the major chronic diseases on the oral health of the elderly and define how the oral health professional can diagnose the oral manifestations of a chronic disease. Material and Methods: To carry out this review article the search strategies included electronic databases, reference lists of articles, and selected textbooks. Articles and textbooks used in this study were mainly reached by using the following keywords: “oral health”, “elderly”, “chronic diseases”, “cardiovascular disease”, “Parkinson’s disease”, “kidney dysfunction”, “metabolic disorders”, “quality of life”. By the end of the research, 34 scientific articles were selected. Results: An inflammation in the oral cavity can trigger a cell and immune response, which will be transmitted by the blood stream to the other components of the individual systems. Parkinson’s disease has repercussions in the oral cavity, not only by motor dysfunction, but also by administered drugs that can lead to xerostomia. Regarding renal impairment, the dental professional must pay special attention primarily to the medication prescribed to the patient, in order not to increase the risk of nephrotoxicity. Apart from this fact, scientific studies indicate a strong association between metabolic disorders and the development of oral diseases Discussion: The weakening of oral health in the elderly mainly occurs when there is a frequent decrease in medical and dental care, neglection of oral hygiene and when there is a systemic pathology responsible for the development of poor oral health conditions. Conclusions: As a result of the heterogeneity of the elderly population, the improvement of oral health should be given according to the diversity of needs required. The frequent presence of systemic diseases and polypharmacy among the elderly, in addition to the possible coexistence of various risk factors, contribute significantly to the development of oral diseases. Thus, in order to enable healthy aging, the dental professional has a key importance in the prevention, diagnosis and treatment of oral diseases among the elderly.
