Abstract(s)
O presente relatório é o culminar do percurso académico inserido no Curso de Mestrado em Enfermagem, com especialização em Enfermagem Médico-Cirúrgica, na área da Pessoa em Situação Crítica, do Instituto de Ciências da Saúde, da Universidade Católica Portuguesa. Tem como objetivo retratar e descrever as experiências vividas durante o período de estágio, sob a forma de reflexão crítica e enquadrá-las no processo de aprendizagem. Este, deve ser analisado com base nas competências reconhecidas ao Mestre e Enfermeiro Especialista, durante a prestação de cuidados de enfermagem integrados à pessoa e família. Estruturalmente encontra-se dividido em dois capítulos distintos, mas complementares, dado o caráter contínuo e sequencial do período de estágio. Tem como temas centrais o papel do Enfermeiro na Prevenção e Controlo de Infeção e no Processo de Doação de Órgãos. O Capítulo I, reflete o percurso realizado até ao momento, onde é feita uma breve menção à experiência profissional anterior e ao período de estágio decorrido no 1ºano do Curso de Mestrado, pela sua importância na definição de objetivos e expectativas para o percurso que agora finda. Em simultâneo, serve de base sustentadora, a partir das qual se evoluiu na reformulação e aprofundamento do conhecimento e pensamento. Por fim, são também exaltadas as competências e habilidades, que estas vivências conferiram ao exercício da profissão. Realça-se aqui, as competências inerentes à gestão de prioridades, resposta atempada e melhoria contínua da qualidade em articulação com a atuação na prevenção e controlo de infeção, através da apresentação de uma proposta de otimização da abordagem a doentes portadores de Carbapenemases resistentes aos Carbapenemos. O Capítulo II, explora o período de estágio, mediante os Domínios de Competência Comuns do Enfermeiro Especialista. Neste ponto, são explanados os objetivos mais pertinentes desenvolvidos durante o contato, associados a uma análise crítico-reflexiva sustentada em fundamentações teóricas, acerca das atividades realizadas. Estas, são em determinadas situações descritas e examinadas sob a forma de casos clínicos e/ exemplos práticos, de forma a facilitar a análise e compreensão. Dentro das principais aprendizagens, destaca-se o desenvolvimento da assertividade da ação e resposta, alicerçadas em competências de juízo critico no desenvolvimento de soluções diferenciadas, no cuidado à pessoa e família. A par, e como principais contributos referem-se os trabalhos realizados nas áreas de Prevenção e Controlo de Infeção e Aprendizagem Profissional, através do desenvolvimento de um instrumento de auditoria para monitorização e caracterização das práticas e da realização de uma Revisão da Literatura, sobre as estratégias utilizadas pelos enfermeiros para melhorar a comunicação com os familiares dos dadores de órgãos, respetivamente. A abordagem aos objetivos e atividades, sob a forma de relatório escrito, permite uma reflexão profunda sobre as competências e habilidades adquiridas durante este percurso, capazes de capacitar e fundamentar a tomada de decisão e consequentemente otimizar e aperfeiçoar a prestação de cuidados, em segurança e qualidade, nas intervenções profissionais futuras. Por fim, fruto da reflexão realizada, sessão levantadas hipóteses relativamente aos próximos objetivos profissionais, numa procura contínua pela diferenciação e perícia profissional.
This report is the culmination of the academic journey within the master’s degree in nursing, specializing in Medical-Surgical Nursing, in the area of the Person in Critical Situation, of the Institute of Health Sciences at the Universidade Católica Portuguesa. It aims portraying and describing the experiences lived during the internship period, in the form of critical reflection and framing them in the learning process. This should be analyzed based on the competencies recognized to the Master and Nurse Specialist, during the provision of integrated nursing care to the person and family. It is structurally divided into two distinct but complementary chapters, given the continuous and sequential nature of the internship period. Its main themes are the role of the Nurse in Infection Prevention and Control and the Organ Donation Process. Chapter I reflects the path taken so far, where a brief mention is made of the previous professional experience and the internship period in the 1st year of the master’s course, due to its importance in defining the objectives and expectations for the course that is now ending. Simultaneously, it serves as a sustaining basis, from which we evolved in the reformulation and deepening of knowledge and thinking. Finally, the competencies and skills that these experiences conferred on the exercise of the profession are also highlighted. We highlight here the skills inherent to the management of priorities, timely response, and continuous quality improvement in articulation with the performance in infection prevention and control, through the presentation of a proposal for optimizing the approach to patients with Carbapenem resistant Enterobacterales. Chapter II explores the internship period through the Domains of Common Competence of the Nurse Specialist. Here, the most relevant objectives developed during the contact are explained, associated with a critical-reflective analysis supported by theoretical foundations about the activities performed. These, in certain situations, are described and examined in the form of clinical cases and/or practical examples, to facilitate analysis and understanding. Among the main learning outcomes, the development of assertiveness of action and response stands out, based on critical judgment skills in the development of differentiated solutions in caring for the person and the family. In addition, and as main contributions, the work carried out in the areas of Infection Prevention and Control and Professional Learning should be mentioned, through the development of an audit tool for monitoring and characterization of practices and the performance of a Literature Review on the strategies used by nurses to improve communication with the relatives of organ donors, respectively. The approach to the objectives and activities, in the form of a written report, allows for a deep reflection on the skills and abilities acquired during this journey, which are capable of empowering and supporting decision-making and, consequently, optimizing and improving the provision of safe and quality care in future professional interventions. Finally, because of the reflections made, hypotheses were raised regarding the next professional goals, in a continuous search for professional differentiation and expertise.
