Abstract(s)
O presente relatório visa espelhar a evolução promovida pelas experiências vivenciadas no estágio realizado durante o Curso de Mestrado com Especialização em Enfermagem Médico-Cirúrgica na Universidade Católica Portuguesa, Instituto de Ciências da Saúde do Porto, na unidade curricular “Estágio Final e Relatório”. O aumento da complexidade tecnológica, informação e consequente aumento da complexidade dos cuidados de saúde tornou imperativo trabalhar a cultura de segurança do doente por forma implementar ações preventivas e melhorar a qualidade dos cuidados prestados. O estágio desenvolveu-se na Unidade de Medicina Intensiva Polivalente e Serviço de Urgência Polivalente de Setembro de 2019 a janeiro de 2020. Fez-se uso de uma metodologia descritiva e reflexiva. Tendo em conta as necessidades encontradas na Unidade de Medicina Intensiva e o tema da segurança e qualidade dos cuidados de enfermagem especializados determinou-se pertinente o desenvolvimento de um Projeto de Melhoria Contínua sobre Práticas Seguras dos Medicamentos de Alerta Máximo que culminou no desenvolvimento de um plano de atividades para translação e implementação da Norma nº 014/2015 por forma a uniformizar práticas. No Serviço de Urgência Polivalente foi identificada a possibilidade de melhoria na adesão à Higiene das Mãos. Como medida simples e eficaz na prevenção e redução das Infeções Associadas aos Cuidados de Saúde foi fundamental o incentivo ao seu cumprimento e Formação das recentes atualizações da Norma. Na reflexão sobre as competências específicas do Enfermeiro especialista em Enfermagem Médico-Cirúrgica à pessoa em situação crítica, o objetivo centrou-se no confronto com a prática por forma analisar e alcançar os padrões de qualidade dos cuidados especializados em Enfermagem Médico-Cirúrgica na pessoa em situação crítica mantendo uma visão humana no Cuidar, reação e ação perante a urgência e emergência cumprindo e sendo elemento-chave na prevenção, intervenção e controlo da infeção e resistência aos antimicrobianos. O trajeto realizado nos estágios e a experiência dos projetos desenvolvidos motivaram reflexão e promoveram a aquisição de novas competências na área do cuidado especializado à pessoa em situação crítica tendo como foco a segurança e qualidade dos cuidados de Enfermagem especializados.
The present report aims to reflect the evolution promoted by the experiences lived in the internship achieve during the Master's Course with Specialization in Medical-Surgical Nursing at the Universidade Católica Portuguesa, Institute of Health Sciences-Porto, in the curricular unit "Final Internship and Report". The increasing technological complexity, new information and consequent increase of health care complexity, made it imperative to work on the patient's safety culture in order to implement preventive actions and improve the quality of the care provided. The internship was developed in Intensive Care Unit and Emergency Service from September 2019 to January 2020. It was used a descriptive and reflective methodology. Taking into account the needs found in the Intensive Medicine Unit and the theme of the safety and quality of specialized nursing care, I thought it was relevant to the development of a Project for Continuous Improvement on Safe Practices of High Alert Medication, this led to the development of an activity plan for translation and implementation of Standard nº. 014/2015 in order to standardize practices. In the Emergency Service, it was taken into account the possibility of improvement in Hand Hygiene Practices. As a simple and effective measure in the prevention and reduction of Infections Associated with Health Care, it was essential to encourage the appliance and training of Standard recent updates. Reflecting about the specific skills of a nurse specialist in Medical-Surgical Nursing to the person in critical situation, the goal was focused on the confrontation of practices in order to analyze it and achieve the quality standards of specialized Care in Medical-Surgical Nursing in critical situation, and above all, being able to maintain a human vision as well as reaction and action before the urgency and emergency fulfilling and being this the key element in the prevention of antimicrobial resistance and healthcare-associated infections intervention and Antimicrobial Resistance and Infection Control. The path made during the internship and the experiences developed by the projects, motivated reflection and promoted the acquisition of new skill and competencies in the area of specialized care to the person in critical situation, always keeping the focus on the safety and quality of specialized nursing care.
