A evolução na área da enfermagem tem vindo a verificar-se cada vez mais com o aprofundamento das competências por parte dos seus profissionais, elevando a qualidade na sua prestação de cuidados.
O curso de especialização em Enfermagem Médico-Cirúrgica do Instituto de Ciências da Saúde da Universidade Católica Portuguesa é particularmente dirigido para a assistência ao doente em estado crítico. O estágio de enfermagem preconizado nesta especialidade tem como objetivo desenvolver competências para a assistência ao doente adulto e idoso com doença grave com base numa enfermagem avançada.
O presente relatório visa a análise descritiva, crítica e reflexiva dos estágios realizados no Serviço de Urgência Geral (módulo I), no Grupo de Coordenação Local do Programa de Prevenção e Controlo de Infeção e Resistência aos Antimicrobianos (módulo III) e na Unidade de Cuidados Intensivos e Intermédios (módulo II) de um hospital em Lisboa. O ponto de partida deste relatório centrou-se no desenvolvimento do projeto de aprendizagem específico para cada estágio, onde foram estabelecidos os objetivos gerais e específicos que se pretendiam alcançar, bem como os indicadores de processo e de resultado.
O primeiro capítulo apresenta a atual evidência científica sobre as boas práticas na prevenção da infeção associada à manipulação do cateter venoso central em unidades de hemodiálise, que culminou na elaboração de um protocolo. O segundo capítulo visa a análise crítico-reflexiva realizada ao longo dos estágios com as atividades e as competências desenvolvidas enquanto (futura) enfermeira especialista.
Os diversos locais de estágio proporcionaram um potencial de aprendizagem, mobilizando conhecimentos, adquirindo competências, e aperfeiçoando o desempenho de forma sustentada. Foi uma experiência trabalhadora mas gratificante, constituindo acima de tudo um momento de aprendizagem por excelência. Através da descrição do contexto de estágio, do processo de desenvolvimento de competências, e da avaliação global e implicações para a prática profissional, foram alcançados os objetivos estabelecidos pelo próprio, e as competências definidas quer pela escola, quer pela Ordem dos Enfermeiros que vão de encontro às exigidas para um enfermeiro especialista.
Developments in the field of nursing have occurred due to the increased skills developed by nurses, raising the quality of care. The specialization course in Medical-Surgical Nursing in Instituto de Ciências da Saúde of Universidade Católica Portuguesa is particularly focused on assisting the patient in a critical condition. The recommended training in this specialty aims to develop skills for assistance to adult and elderly patients with severe disease based on an advanced level of nursing. This report aims to be a descriptive, critical and reflective analysis of internships in the General Emergency Service (module I), the Local Coordination Group of Prevention and Infection Control and Antimicrobial Resistance (module III) Intensive Care Unit and Intermediate Care Unit (module II) of a Lisbon hospital. The starting point of this report focused on the development of specific learning project for each stage, the general and specific objectives to be obtained as well as the process and outcome indicators which have been established. The first chapter presents the current scientific evidence on the best practice in the prevention of infection associated with handling central venous catheter for hemodialysis units, which culminated with a protocol. The second chapter is a critical and reflective analysis during the training period with the activities and skills developed as a (future) specialist nurse. The different training facilities provided a learning potential, mobilizing knowledge, acquiring skills, and improving the sustained performance. It was a hard-working period but a rewarding experience, it was above all a moment of learning par excellence. By describing the training context, the skills development process, and the overall assessment and implications for professional practice, the objectives set by myself have been achieved, as well as the competencies defined either by the school or by the Ordem dos Enfermeiros required for a specialist nurse.
