A elaboração do relatório surge no âmbito do curso de Mestrado em Enfermagem com Especialização Enfermagem Médico-Cirúrgica, da Escola de Enfermagem da Universidade Católica Portuguesa, em Lisboa, e visa traduzir o meu percurso da Unidade Curricular Estágio, subdividido em três partes Serviço de Urgência, Cuidados intensivos e Grupo de Coordenação Local Programa de Prevenção e Controlo das Infeções e da Resistência aos Antimicrobianos. O Estágio I foi realizado no Serviço de Urgência Geral destacando-se a formação realizada sobre a prevenção da pneumonia associada ao ventilador, abordando a problemática da higiene oral e a opção da intubação com sonda gástrica via oral, da pessoa submetida a ventilação mecânica invasiva. O Estágio II foi realizado na Unidade de Cuidados Intensivos e Intermédios, onde se sublinha a prestação de cuidados especializados à pessoa em situação critica e sua família, mais concretamente à pessoa submetida a ventilação mecânica invasiva mantendo a problemática da prevenção da pneumonia associada ao ventilador. Aqui realizei auditorias incidindo sobre os feixes de intervenções de prevenção pneumonia associada ao ventilador/intubação, presentes na Norma 21/2015 da Direção Geral da Saúde. No término do estágio foram apresentados os resultados das auditorias realizadas à equipa de Enfermeiros. No Grupo de Coordenação Local Programa de Prevenção e Controlo das Infeções e da Resistência aos Antimicrobianos, propusemo-nos realizar uma revisão sistemática da literatura sobre “A escovagem dos dentes associada à clorohexidina: Evidência na prevenção da Pneumonia Associada ao Ventilador”. Este estágio possibilitou o desenvolvimento de competências no âmbito da prevenção e controlo das infeções associadas aos cuidados de saúde. A transversalidade ao longo da realização dos três Estágios visou uma intervenção e desenvolvimento progressivo de autonomia na prestação de cuidados sendo que os diferentes contextos promoveram o desenvolvimento de competências na minha área de especialização.
The elaboration of this report is part of the Master's program in Nursing with Specialization in Medical and Surgical Nursing, from the Nursing School of the Portuguese Catholic University, in Lisbon, and aims to show my progress during my internship, which consists of three parts, Emergency Service, Intensive Care and Option, each one with a total of 180 hours. The first part of the internship was at the General Emergency Service, highlighting the training carried out on the prevention of ventilator-associated pneumonia, addressing the issue of oral hygiene and the option of intubation with oral gastric probe of the person submitted to invasive mechanical ventilation. The second part was carried out in the Intensive and Intermediate Care Unit, which emphasizes the specialized care to the person in critical situation and his family, more specifically to the person submitted to invasive mechanical ventilation maintaining the problem of ventilator-associated pneumonia. During this internship I conducted audits to the performing of the nurse team on the intervention bundles ventilator-associated pneumonia, present in the 21/2015 Norm of the General Directorate of Health. The results were presented at the end of this second internship. The last internship was carried out in the Local Coordination Group Prevention and Control Program for Antimicrobial Infections and Resistance, where we decided to carry out a systematic review of the literature on "Chlorhexidine-associated toothbrushing: Evidence for the prevention of Ventilator-Associated Pneumonia ". This internship part of the curricular unit, allowed me to develop competences in the field of prevention and control of infections associated with health care. The aim for this internship was accomplish by the increased of autonomy in the care delivery of prevention of ventilator-associated pneumonia in different contexts.
