No
Introduction: Periapical healing after endodontic treatment is traditionally assessed using clinical and radiographic criteria. However, these methods primarily provide morphological information and may not accurately reflect the initial biological activity of periapical tissues. In this context, biomarkers have been studied as potential indicators of inflammation, tissue remodeling, and bone regeneration. Objective: To map and summarize the available scientific evidence on biomarkers studied as potential indicators of periapical healing and bone regeneration after endodontic treatment, identifying the biomarkers evaluated, the biological matrices used, the analytical methodologies applied, and the main
results published in the literature. Methods and Materials: A Scoping Review was conducted according to the PRISMA-ScR guidelines. The literature search was performed in the Dentistry & Oral Sciences Source, MEDLINE, Scopus, and Web of Science databases. The identified records were imported into Rayyan, where duplicates were removed and studies were selected through title and abstract screening, followed by full text review. Human studies evaluating biomarkers associated with periapical healing and/or bone regeneration after endodontic treatment were included.
Results: Eleven studies published between 2012 and 2025 were included. The most frequently studied biomarkers were inflammatory mediators, such as IL-1β, IL-6, IL-8, TNF-α, IL-10, and PGE2, and matrix metalloproteinases, particularly MMP-2, MMP-8, and MMP-9. Biomarkers related to angiogenesis, tissue repair, and bone metabolism were also identified, including VEGFA, ANGPT1, CTGF, COL1A1, VTN, ITGA5, FGF-23, and OPG. The main biological matrices used were serum, saliva, gingival crevicular fluid, periapical fluid, periradicular tissue fluid, and periapical tissue or granulomas. Laboratory methods included ELISA, multiplex immunoassay, qRT-PCR, zymography, immunohistochemistry, and biochemical analyses. Conclusions: The biomarkers analyzed appear to provide complementary information to the clinical and radiographic evaluation of periapical healing. However, the available evidence does not yet support the identification of a universally validated biomarker panel for the clinical monitoring of periapical bone regeneration. The heterogeneity of study designs, biological matrices, laboratory methods, and follow-up periods underscores the need for future studies with standardized protocols, larger sample sizes, and a systematic correlation between biomarkers
and clinical-radiographic outcomes.eng
