Abstract(s)
No âmbito do Curso de Mestrado em Enfermagem, na área de Especialização em Enfermagem Médico-Cirúrgica, surge a elaboração do presente relatório com o objetivo de descrever as competências desenvolvidas/adquiridas através de variadas atividades concretizadas não só durante o percurso profissional, em contexto de trabalho nos Cuidados Intensivos, como ao longo da formação pós-graduada, em contexto de Serviço de Urgência e Serviço de Anestesiologia, tendo em vista a obtenção do título de Enfermeira Especialista em Enfermagem Médico-Cirúrgica.
Por esta razão, o trabalho final assenta numa metodologia crítico-reflexiva na e sobre a prática clínica. Compreende ainda a conceptualização teórica nos mais variados contextos da praxis clínica, tendo em vista o desenvolvimento da enfermagem enquanto disciplina. Considera-se a Teoria da Transição de Meleis (2010), o suporte conceptual principal deste relatório, na medida em que coloca a pessoa na centralidade dos cuidados de enfermagem, que em interação com o ambiente, necessita de um reforço para vivenciar de forma positiva o processo de transição saúde/doença. Defino a minha intervenção na aquisição de competências de Enfermeira Especialista, na perspetiva de responder à Pessoa e sua Família, com intervenções de enfermagem classificadas no seu resultado, como ações que promovem, que recuperam ou facilitam a saúde da díade Pessoa/Família.
O tema transversal do relatório diz respeito às intervenções pelo enfermeiro na abordagem da via aérea à pessoa em situação crítica, que aliás motivou muitas das atividades desenvolvidas nos estágios, principalmente, enquanto formadora devidamente fundamentada pela realização da Revisão Sistemática da Literatura que me permitiu conhecer o “estado da arte” sobre o tema.
Relata os cuidados de enfermagem prestados à pessoa em situação crítica, mantendo as funções básicas, prevenindo lesões e aumentando o potencial de recuperação da pessoa, durante todo o processo anestésico e, também em contexto de urgência. Evidencia as dificuldades encontradas em momentos de aprendizagem e de reflexão, as estratégias de acolhimento e comunicação com a pessoa e sua família; expõe a importância da prática baseada na evidência, através da análise de metodologias do trabalho de enfermagem, dinâmicas das equipas de enfermagem, elaboração e análise de normas, protocolos e procedimentos, promovendo a qualidades dos cuidados de saúde e práticas seguras.
Este relatório engloba três perspetivas diferentes, em três diferentes contextos de cuidados de enfermagem, estabelecendo uma abordagem direta sobre a intervenção do Enfermeiro Especialista. Este é visto como um modelo de ação e um agente de mudança a seguir na abordagem avançada à Pessoa/Família e, finalmente, a ser tido em conta como uma referência na prestação de cuidados de excelência.
As part of Masters Course in area of Specialization in Medical-Surgical Nursing, comes the preparation of this report in order to describe the skills developed/acquired through various activities implemented not only for the career in context working in intensive care units, as throughout postgraduate training in the context of Emergency Room and Anesthesiology Department, to obtaining the title of Nurse Specialist in Medical-Surgical Nursing. The final work is based on a critical-reflexive methodology on practice. It also includes the theoretical conceptualization in various contexts of clinical praxis, and development of nursing as a discipline. It is considered the Theory of Transition Meleis (2010), the main conceptual support of this report, in that it puts the person on the centrality of nursing care, which in interaction with the environment, you need a booster for a positive experience the transition process health/disease. I define my intervention in skills Nurse Specialist in perspective responding to the Person and his Family, with nursing interventions that can be classified in its outcome, as actions that promote, facilitate or recovering health dyad Person/Family. The theme of the report relates to interventions by nurses in airway management in critical ill patients. This fact has motivated many of the activities on stage, while forming and the reasons for conducting the Systematic Review of the Literature that helped identify the "state art". Reports the nursing care provided to a person in critical condition, while maintaining the basic functions, preventing injuries and increasing the potential for recovery of the person throughout the anesthetic process and also in the context of urgency. It highlights the difficulties encountered in moments of learning and reflection, within the strategies and host communication with the person and their family; exposes the importance of evidence-based practice, by analyzing methodologies of nursing work, dynamics of teams nursing, drafting and analysis of standards, protocols and procedures, promoting quality of care and safe practices.
