Abstract(s)
O presente relatório surge no âmbito da avaliação no plano de estudos do 7º Curso de
Mestrado em Enfermagem com Especialização em Enfermagem Comunitária, do
Instituto de Ciências da Saúde da Universidade Católica Portuguesa. Ele integra a
exposição do percurso realizado, traduzindo por fases o conjunto de todas as atividades
desenvolvidas. Perceção, análise e ação formam assim um fio condutor que justifica as
estratégias destacadas e utilizadas para fazer face aos problemas que foram surgindo.
Assim, o módulo I corresponde à fase de Diagnóstico de Saúde onde foi elaborado um
instrumento de recolha de dados e posteriormente aplicado numa população selecionada
por conveniência na abrangência da Unidade de Cuidados na Comunidade (UCC) de
Campanhã. Esta primeira fase definiu como áreas de intervenção a Gestão e Adesão ao
Regime Medicamentoso.
A Intervenção Comunitária refere-se ao módulo II onde os quatro utentes alvo desta
intervenção foram referenciados pela UCC de Campanhã e distribuídos aleatoriamente e
no final foi possível afirmar que em todos os utentes abrangidos pela intervenção foram
obtidos ganhos em saúde. Por fim o módulo III, de caráter opcional, desenvolveu-se na
Equipa de Cuidados Continuados Integrados (ECCI) de Campanhã e na Unidade de
Cuidados de Saúde Personalizados (UCSP) de São Roque da Lameira de forma a
desenvolver conhecimentos e competências no acompanhamento e na assistência do
individuo nos seus processos de vida, tendo culminado na elaboração de uma norma para
a UCC/ECCI de Campanhã sobre as Superfícies de Apoio.
A metodologia exposta passa essencialmente pela exploração de evidências estatísticas
resultantes dos formulários realizados junto da população considerada. Após uma
caracterização da amostra aleatória inquirida, o processamento estatístico permitiu
evidenciar tendências e correlações não paramétricas consideradas relevantes e
explicitadas no presente documento.
Num contexto mais global e para finalizar, uma reflexão crítica aborda e sintetiza os
objetivos traçados e respetivos resultados, as dificuldades sentidas e as competências
desenvolvidas no que rumo à Especialidade em Enfermagem Comunitária diz respeito.

It integrates the exposure of paid route, resulting in phases the set of all activities performed. Perception, analysis and action thus form a common thread that justifies the strategies highlighted and used to address the problems that arose. Thus, the module I corresponds to Health Diagnostic phase where a data collection instrument was developed and then applied to a population selected by convenience in the scope of the Community Care Unit (CCU) of Campanhã. This first phase defined as areas of intervention Management and Adherence to Medication Regime. The Community Intervention refers to the module II where the four target users of this intervention were referred by UCC campaign is randomly distributed and at the end it was possible to say that in all users covered by the intervention were obtained health gains. Finally the III module, optional character, developed in the Team Continuous Care (ECCI) Campanhã and Personalized Health Care Unit (UCSP) of São Roque da Lameira in order to develop knowledge and skills in monitoring and the individual's assistance in their life processes, culminating in the development of a standard for UCC / ECCI campaign on the Surface Support. The methodology set out passes essentially the exploitation of statistical evidence resulting from the forms made with the population considered. After a characterization of the surveyed a random sample, the statistical processing has highlighted nonparametric trends and correlations considered relevant and explained in this document In a global context and to finish, a critical reflection approaches and summarizes the strokes and their outcomes objectives, these difficulties and skills developed in the course Specialists in Community Nursing concerns.
