O presente relatório surge no âmbito do Mestrado de natureza profissional da área da Enfermagem Médico-Cirúrgica, com especialização na área da Pessoa em Situação Crítica. Este pretende demonstrar todo o percurso realizado, defendendo a aquisição das Competências Específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúgica e das Competências de Mestre. O estágio foi realizado no Serviço de Urgência de um Centro Hospitalar de Lisboa, com duração de 360 horas de contato, em que os objetivos específicos definidos foram prestar cuidados especializados à pessoa em situação crítica e a sua família, e contribuir para as boas práticas da equipa de enfermagem na gestão do erro terapêutico. Incluída ainda neste documento a revisão de escopo, realizada sobre o tema “Intervenções do Enfermeiro na prevenção do Erro Terapêutico numa Unidade de Cuidados Intensivos: uma Scoping Review”, com o intuito de mapear a literatura, com base no modelo preconizado pelo Joanna Briggs Institute para extração de dados. Como resultado obteve-se uma relevante incidência dos erros terapêuticos devido à complexidade de uma Unidade de Cuidados Intensivos, concluindo-se que os enfermeiros têm um papel fundamental para a prevenção dos erros terapêuticos através da melhoria do ambiente de trabalho – ecologia restaurativa, gestão dos medicamentos Look Alike Sound Alike e Alto Risco e trabalho em equipa; uso de tecnologia – informatizando os protocolos clínicos; compreensão do processo cognitivo na realização de tarefas- através de checklists, padronizações, metacognição e evitando a multitarefa; sublinha-se ainda a importância da notificação de incidentes.
This report is part of the professional Master's in the area of Medical-Surgical Nursing, with specialization in the area of Persons in Critical Situation. This one intends to demonstrate the entire path taken, defending the acquisition of the Specific Skills of the Specialist Nurse in Medical-Surgical Nursing and of the Master's Skills. The internship was carried out in the Emergency Department of a Hospital Center in Lisbon, with a duration of 360 hours of contact, in which the specific objectives defined were to provide specialized care to the person in a critical situation and their family, and to contribute to the good practices of the nursing team in the management of therapeutic error. Also included in this document is the scope review, carried out on the theme "Nurses' Interventions in the prevention of Therapeutic Error in an Intensive Care Unit: a Scoping Review", in order to map the literature, based on the model recommended by the JBI for extracting of data. As a result, a relevant incidence of therapeutic errors was obtained due to the complexity of an Intensive Care Unit, concluding that nurses have a fundamental role in the prevention of therapeutic errors through the improvement of the work environment - restorative ecology, management of LASA and MAR medicines and teamwork; use of technology – computerizing clinical protocols; understanding of the cognitive process in performing tasks - through checklists, standardization, metacognition and avoiding multitasking; the importance of incident reporting is also underlined.
