No contexto organizacional, a liderança implica interação entre vários indivíduos e pressupõe capacidade de exercer influência sobre os mesmos, no sentido de se alcançarem os objetivos pessoais/profissionais/organizacionais. O estudo que se apresenta inscreve-se no paradigma de investigação quantitativo e adota um caráter analítico, correlacional e transversal. Assume como principal objetivo, analisar a relação existente entre a qualidade da interação líder membro, o empenhamento organizacional e profissional, e a qualidade do exercício profissional dos enfermeiros em contexto hospitalar. Os instrumentos utilizados foram: Escala LMX-7 versão para colaboradores (LMX) e versão para chefia (CLMX); Escala de empenhamento organizacional; Attitudinal Commitment Scale (ACS) e Satisfação dos Utentes com os Cuidados de Enfermagem no Hospital (SUCEH21). A qualidade de exercício profissional operacionalizou-se através dos seguintes indicadores de qualidade: Taxa de avaliação do risco de úlceras por pressão (UPP); Taxa de avaliação do risco de queda; Taxa de avaliação sistemática da dor e Satisfação dos utentes com os cuidados de enfermagem. Adotou-se a Teoria da Consecução dos Objetivos desenvolvida por Imogene King como marco teórico subjacente a esta investigação, na medida em que o foco de atenção da temática se situa ao nível do sistema interpessoal e do sistema social. Foi realizado um estudo preliminar com uma amostra de enfermeiros 161 em contexto de formação com o objetivo testar o comportamento dos instrumentos de recolha de dados que se iriam utilizar no estudo. O estudo desenvolveu-se numa amostra intencional constituída por 15 unidades de internamento de um Centro Hospitalar, em que todos os enfermeiros chefes e enfermeiros foram convidados a participar, bem como todos os utentes internados nessas unidades, durante três meses consecutivos. Constituiram-se 342 díades (enfermeiros/enfermeiros-chefes) e obteve-se uma amostra de 273 utentes. Contou- se ainda, com as Taxas supracitadas dessas unidades durante os mesmos meses. Os resultados revelam que a liderança relacional se encontra correlacionada positivamente com todas as componentes do empenhamento organizacional, no entanto com diferentes intensidades, sendo as mais relevantes as componentes afetiva e normativa, e com o empenhamento profissional. Quanto às variáveis que influenciam positivamente a qualidade do exercício profissional verificou-se que o CLMX foi a única com a capacidade de influenciar individualmente o mesmo. No entanto, quando o LMX se associa ao CLMX, às componentes afetiva e normativa e à ACS; e a ACS se associa às componentes afetiva e normativa parece verificar-se uma influência positiva no exercício profissional. Demonstra-se assim, que a qualidade dos cuidados de enfermagem oferecidos à população depende da interação e relacionamento entre enfermeiro e enfermeiro chefe, e do empenhamento dos enfermeiros. Assim, afigura-se de extrema importância a necessidade dos enfermeiros chefes estabelecerem uma relação de parceria com os enfermeiros, e do interesse da organização reconhecer, valorizar e promover o desenvolvimento dos mesmos.
In the organizational context, leadership implies interaction between several individuals and assumes ability to exert influence on them, in order to achieve personal/professionals/organizational goals. The empirical research was based on analytical, correlational and cross-sectional orientation and on a quantitative approach. The aim of this study is to analyse the relation between the quality of the leader- member exchange, the organizational and professional commitment, and the quality of the professional exercise of nurse in hospital context. The instruments used were: LMX-7 Scale member version (LMX) and leader version (CLMX); Organizational commitment scale; Attitudinal Commitment Scale (ACS) and customers' satisfaction with nursing care in the Hospital (SUCEH21). The quality of the professional exercise operationalized through the follow indicators of quality: risk of pressure ulcers rate (UPP); risk of patient fall rate; systematic evaluation of pain rate, and satisfaction from costumers with nursing care. Imogene King’s Goal Attainment Theory was assumed as theoretical framework underlying this research, to the extent that the focus of attention of the subject lies on the level of interpersonal and social system. A preliminary study was conducted with a sample of 161 student nurses with the objetive of testing the behavior of the data collection instruments that would be used in the study. The study was developed in an intentional sample consisting of 15 hospital units of a Hospital Centre, where all the nurses and head nurses were invited to participate, as well as all patients admitted into these units, for three consecutive months. 342 dyads were formed (nurses/head nurses) and obtained a sample of 273 patients. There were also featured the above-mentioned Rates of these units during the same months. The results reveal that the relational leadership is positively correlated with all components of organizational commitment, however with different intensities, being the most relevant the affective and normative components, and with the professional commitment. CLMX was the only one with the ability to individually influence positively the quality of the professional exercise. However, when the LMX is associated to CLMX, to affective and normative components and to ACS; and the ACS is associated to the affective and normative components it seems to exist a positive influence in the professional exercise. It is concluded that the quality of nursing care provided to the population depends on the interaction and relationship between nurse and head nurse, and the commitment of nurses. Therefore it is extremely important the head nurse establish a relationship based on partnership with the other nurses, and the interest of the organization to recognize, enhance and promote the development of these professionals.
