Este relatório descreve todo o processo de aquisição de conhecimentos e competências especializadas adquiridas em Enfermagem Médico-cirúrgica. Para a prestação de cuidados à Pessoa em Situação Critica, o enfermeiro especialista nesta área, detém competências científicas, éticas, relacionais e técnicas e coloca-as em prática na prestação de cuidados, mas não só, pois também adquire competências de gestão, formação e investigação. Assim, encontra-se ao longo deste trabalho a descrição das atividades por mim desenvolvidas e as competências especializadas, às quais me dediquei e foram adquiridas ao longo da prática clínica realizada em dois contextos principais: na SIV (Suporte Imediato de Vida) e no SMI (Serviço de Medicina Intensiva). As situações vivenciadas ao longo dos estágios englobaram momentos de criticidade e grande vulnerabilidade onde a Pessoa em Situação Critica esteve em destaque. A monitorização e vigilância constante fizeram com que se desenvolvessem competências não só técnicas ou teóricas, mas também, emocionais, comunicacionais e de promoção da relação e de conforto e qualidade de vida, aspetos que definem a profissão como essencial no que diz respeito ao cuidado na pessoa em situação critica. O tema escolhido para ser desenvolvido ao longo deste período não poderia ser mais pertinente e tenta-¬se que seja incluído de forma transversal ao longo de todo o relatório, pois demonstra como a atitude do enfermeiro especialista leva a uma melhor prestação dos cuidados de enfermagem especializados. A Decisão de Não Reanimação (DNR) como decisão multidisciplinar fará refletir qual a importância de integrar todos os envolvidos na vida do paciente e na prestação de cuidados numa decisão comum e como isso pode contribuir para uma melhor decisão de saúde. Uma decisão interdisciplinar, ética, responsável e ponderada. De acordo com os processos de revisão de literatura, foram identificados os intervenientes na questão da decisão de não reanimação e justificada a sua imposição no processo de decisão, posteriormente, justifica-se o porquê da importância de uma auscultação multidisciplinar e os ganhos existentes em saúde, aquando desta decisão são tomadas tendo em atenção todos os aspetos que integram esta problemática – Decisão de não reanimação.
This report describes the entire process of skills and knowledge acquired and specialized in Medical Surgical Nursing. To provide care to the critical patient, it is for the specialist nurse in this area, mandatory to develop not only scientific, technical, ethical, and relational skills but it is also important to expand the knowledge in management, training and research skills. Therefore, throughout this compendium it is possible to find the description of the activities and the specialized skills, developed by myself and mainly acquired in two different placements: At ILS (Immediate Life Support) and at ICU (Intensive Care Unit). Over time, it was identified vulnerable and critical situations where vigilance and monitoring were constant so, communicational and emotional skills were highly developed in order to promote comfort, safeness and life quality to the patients. The topic I have chosen to be developed over this period could not be more relevant and it is included in a transversal way throughout the report, since it is demonstrated how the specialist nurse's attitude leads to a better care. The Do Not Attempt CardioPulmonary Resuscitation (DNACPR) or in Portuguese the Decisão de Não Reanimação (DNR) as a multidisciplinary decision, will reflect on the importance of getting everyone involved in patient's life and in his care and that will lead to a better and healthier decision An interdisciplinary, ethical, responsible and thoughtful decision. According to the literature review, those involved in the non-resuscitation decision were indicted and justified its imposition in the decision process, following that, it was justified why the importance of multidisciplinary decisions and the success of that in patients’life.
