Abstract(s)
O presente relatório de estágio enquadra-se no âmbito do 13º Mestrado da Universidade Católica Portuguesa - Instituto de Ciências da Saúde em Enfermagem com Especialização em Enfermagem Médico-cirúrgica, na área de especialização à Pessoa em Situação Crítica. O presente relatório reflete as vivências, descreve as experiências, a aquisição de competências e os conhecimentos mais significativos ao longo desta caminhada académica e de prática clínica. Através da realização deste relatório, procuramos dar visibilidade às soluções desenvolvidas para as situações identificadas, exercer uma prática reflexiva das atividades desenvolvidas, aliando a investigação e procura incessante de novas evidências, que pudessem realçar a díade da teoria – prática e assim obter ganhos em saúde. Os cuidados de enfermagem partiram da premissa de Enfermagem especializada, com base na evidência científica, na gestão de cuidados diferenciados, na consciência crítica para os problemas da prática, mas de igual modo assentes na relação terapêutica e processos de comunicação. Pelo que foi imprescindível aliar referenciais teóricos, como a Teoria das Necessidades Fundamentais de Virgínia Henderson e a Teoria das Transições de Afaf Meleis. O relatório abrange dois períodos de estágio realizados em diferentes contextos: Serviço de Urgência e Unidade de Cuidados Intensivos. A escolha dos locais de estágio prendeu-se pelos objetivos delineados pelo curso e pela ambição de compreender mais profunda e detalhadamente, através da observação, experiência do percurso da Pessoa em Situação Crítica, aliados á temática escolhida, sobre as Intervenções da Equipa de Enfermagem no Processo de Doação de Órgãos. Em conjunto, ambos os locais de estágio foram de grande interesse e enriquecedores pelo crescente corpo de conhecimentos, mobilização de saberes, habilidades e competências. A escolha da temática, deveu-se ao interesse pessoal em investigar e desenvolver competências especializadas nessa área, ao questionar quais as intervenções de que o enfermeiro é detentor no seu exercício de autonomia e interdisciplinaridade. Através de uma Revisão Scoping da Literatura, permitiu o desenvolvimento de competências no campo da investigação, bem como a partilha dos dados analisados, através da divulgação dos resultados. O papel desempenhado pela equipa de enfermagem no decurso do processo de doação de órgãos é crucial. Requer uma praxis baseada em conhecimentos técnico-científicos, habilidades e competências específicas para a identificação do potencial dador, após confirmação e diagnóstico de morte cerebral, manutenção fisiológica do potencial dador, notificação do gabinete de transplante, bem como habilidade na comunicação terapêutica e de más notícias, sendo condutas fundamentais na ética e legislação vigente. A relação/ interação entre a equipa de enfermagem e os familiares, são determinantes na tomada de decisão, confiança e segurança em todo o processo.
This internship report falls within the scope of the 13th Master's Degree at the Catholic University of Portugal - Institute of Health Sciences in Nursing with Specialization in Medical-Surgical Nursing, in the area of specialization for People in Critical Situation. This report reflects the experiences, describes the experiences, the acquisition of skills and the most significant knowledge throughout this academic and clinical practice journey. Through the realization of this report, it was possible to give visibility to the solutions developed for the situations identified, to exercise a reflexive practice of the activities developed, combining the investigation and unceasing search for new evidence, which could enhance the dyad of theory - practice and thus obtain gains in Cheers. Nursing care started from the premise of Advanced Nursing, based on scientific evidence, on the management of differentiated care, on critical awareness of the problems of practice, but equally based on the therapeutic relationship and communication processes. So it was essential to combine theoretical references, such as Virginia Henderson's Theory of Fundamental Needs and Afaf Meleis's Theory of Transitions. The report covers two internship periods carried out in different contexts: the Emergency Department and the Intensive Care Unit. The choice of internship locations was due to the objectives outlined by the course and the ambition to understand more deeply and in detail, through observation, experience of the path of the Person in Critical Situation, allied to the chosen theme, about the Nursing Team Interventions in the Organ Donation Process. Together, both internships were of great interest and enriched by the growing body of knowledge, the mobilization of knowledge, skills and competences. The choice of the theme was due to the personal interest in investigating and developing specialized skills in this area, by questioning which interventions the nurse has in his exercise of autonomy and interdisciplinarity. Through a Scoping Review of Literature, it allowed the development of skills in the field of research, as well as the sharing of the analyzed data, through the dissemination of results. The role played by the nursing team during the organ donation process is crucial. It requires a praxis based on technical and scientific knowledge, specific skills and competences for the identification of the potential donor, after confirmation and diagnosis of brain death, physiological maintenance of the potential donor, notification of the transplant office, as well as ability in therapeutic and poor communication. news, being fundamental conducts in ethics and current legislation.
