No âmbito do curso de Mestrado em Enfermagem com Especialização em Enfermagem Médico-Cirúrgica na área de Enfermagem à Pessoa em Situação Crítica, no Instituto de Ciências da Saúde da Universidade Católica Portuguesa de Lisboa, foi elaborado o presente relatório de estágio. Este relatório, tem como objetivo descrever e refletir acerca de todo o percurso académico e analisar o processo de desenvolvimento de aquisição de competências comuns e específicas de enfermeira especialista, assim como obtenção de grau de mestre. Este documento encontra-se dividido em três capítulos: no primeiro capítulo será apresentado uma scoping review sobre fatores de risco que contribuem para a auto-extubação da pessoa adulta em unidade de cuidados intensivos. Foram identificados 13 fatores de risco, agrupados em 3 categorias diferentes: fatores relacionados com a pessoa, com o “desmame” ventilatório e relacionados com os profissionais de saúde e ambiente. No segundo capítulo é realizada a descrição dos contextos de estágio onde foram realizados a aquisição e desenvolvimento de competências, nomeadamente o serviço de urgência e unidade de cuidados intensivos. No decurso do terceiro capítulo, é exposto uma descrição e reflexão das vivências e das atividades desenvolvidas nos contextos de estágios do capítulo anterior e no percurso da prática profissional, com a finalidade de aquisição de competências comuns e específicas de enfermeiro especialista em enfermagem médico-cirúrgica à pessoa em situação crítica, assim como as competências de mestre através da elaboração da scoping review do capítulo primeiro. Durante este percurso académico, foi possível uma aprendizagem e consolidação de conhecimentos técnico-científicos assim como de competências de enfermeiro especialista e mestre, para uma prática baseada em evidência científica, uma melhoria da qualidade na prestação de cuidados e uma tomada de decisão mais fundamentada ao longo do meu futuro percurso profissional.
This report was prepared within the scope of the Master's Degree in Nursing with Specialization in Medical-Surgical Nursing in the area of Critical Care Nursing, at the Instituto de Ciências da Saúde da Universidade Católica Portuguesa of Lisbon. This report aims to describe and reflect on the entire academic journey and analyze the process of development of the acquisition of common and specific competencies of a specialist nurse, as well as the achievement of a master's degree. This document is divided into three chapters: the first chapter presents a scoping review on risk factors contributing to self-extubation in adults in intensive care units. Thirteen risk factors were identified, grouped in 3 different categories: factors related to the person, to ventilator "weaning" and related to health care professionals and environment. The second chapter describes the internship settings where the acquisition and development of skills took place, namely the emergency department and the intensive care unit. In the third chapter, a description and reflection of the experiences and activities developed in the internship contexts of the previous chapter and in the course of professional practice are presented with the purpose of acquiring the common and specific competencies of a nurse specialist in medical-surgical nursing to critically-ill patients, as well as the master's competencies through the development of the scoping review of the first chapter. During this academic journey, it was possible to learn and consolidate technical and scientific knowledge, as well as the skills of specialist and master nurses, for a practice based on scientific evidence, to improve the quality of care provision, and to make more informed decisions throughout my future professional career.
