Barreiras à prevenção de úlceras por pressão em unidades de cuidados intensivos: um protocolo de scoping review

Introdução: As úlceras por pressão constituem um problema persistente de elevada relevância clínica, com impacto significativo no bem-estar e na qualidade de vida das pessoas afetadas. A sua prevenção em contextos de saúde, particularmente nas unidades de cuidados intensivos, configura um processo complexo, influenciado não apenas pelo conhecimento e pelas atitudes dos enfermeiros, mas também por múltiplos fatores ambientais e organizacionais. Compreender as barreiras que dificultam a implementação eficaz de medidas preventivas nestes contextos é, portanto, fundamental para orientar estratégias que melhorem a prática clínica e os resultados em saúde. Objetivo: Mapear a evidência científica disponível sobre as barreiras à prevenção de úlceras por pressão, na pessoa em situação crítica, internada numa unidade de cuidados intensivos. Métodos: Será seguida a metodologia mais recente proposta pelo Joanna Briggs Institute para scoping reviews. Serão incluídos estudos que envolvam, como participantes, pessoas em situação crítica; como conceito, as barreiras à prevenção de úlceras por pressão; e como contexto, unidades de cuidados intensivos. A pesquisa será realizada nas bases de dados Academic Search Complete, CINAHL Complete, MedicLatina, MEDLINE Complete, PubMed e SCOPUS, bem como em fontes de literatura cinzenta selecionadas. A triagem dos títulos e resumos será efetuada de forma independente por dois revisores, seguindo-se a avaliação dos textos integrais. Os dados extraídos serão analisados e apresentados de forma descritiva, recorrendo a síntese narrativa e, sempre que pertinente, a representações tabulares e/ou gráficas.
Introduction: Pressure ulcers remain a persistent and significant clinical problem, with a substantial impact on the well-being and quality of life of affected individuals. Prevention in healthcare settings, particularly in intensive care units, is a complex process influenced not only by nurses’ knowledge and attitudes but also by multiple environmental and organizational factors. Understanding the barriers that hinder the effective implementation of preventive measures in these contexts is essential to guide strategies that improve clinical practice and health outcomes. Objective: To map the available scientific evidence on barriers to the prevention of pressure ulcers in critically ill people admitted to an intensive care unit. Methods: This scoping review will follow the most recent methodology proposed by the Joanna Briggs Institute. Eligible studies will include, as participants, critically ill patients; as the concept, barriers to pressure ulcer prevention; and as the context, intensive care units. The search will be conducted in Academic Search Complete, CINAHL Complete, MedicLatina, MEDLINE Complete, PubMed, and SCOPUS, as well as selected sources of grey literature. Titles and abstracts will be screened independently by two reviewers, followed by full-text assessment of potentially eligible studies. Extracted data will be analyzed and presented descriptively through narrative synthesis and, where appropriate, in tabular and/ or graphical formats.
