Cortical bone thickness of the anterior maxilla using AI segmentation

Introduction: The thickness of the buccal cortical plate in the anterior maxilla is a critical factor in implantology, orthodontics, and aesthetic dentistry. Cone-beam computed tomography CBCT combined with artificial intelligence (AI)–based segmentation and three-dimensional digital workflows, may facilitate three-dimensional assessment of alveolar bone morphology. Aim: To evaluate buccal cortical bone thickness in the anterior maxillary region using CBCT imaging associated with AI-based segmentation and three-dimensional digital analysis. Materials and Methods: CBCT examinations of patients with anterior maxillary teeth were retrospectively analysed. DICOM files were imported into CoDiagnostiX® software for AI-based segmentation and STL model generation. The obtained models were analysed in Geomagic Control X® through a standardized three-dimensional workflow. Linear measurements of cortical bone thickness were performed at the cervical, middle, and apical thirds of the teeth. Descriptive, inferential, and exploratory statistical analyses were conducted to assess cortical bone distribution and anatomical variability. Results: A total of 44 CBCT examinations were analysed. Considerable anatomical variability was observed in buccal cortical bone thickness. Mean thickness values generally decreased from the apical to the cervical level, with the lowest values observed at the cervical third. Mixed linear model analysis demonstrated a significant effect of root level on cortical bone thickness on both the right (p = 0.003) and left sides (p = 0.042), with significantly greater thickness at the apical level compared with the cervical level (Bonferroni-adjusted p = 0.002). No significant effects of age or sex were identified. On the left side, lateral incisors presented significantly greater cortical thickness than central incisors (p = 0.006). Conclusion: The anterior maxillary region presents a predominance of thin buccal cortical bone associated with considerable anatomical variability. The present study demonstrated the feasibility of a three-dimensional digital workflow that combines CBCT imaging, AI-assisted segmentation, STL model generation, and three dimensional morphometric analysis to assess alveolar bone morphology. The findings of the present study may support more individualized treatment planning in implantology, orthodontics, and aesthetic dentistry.
Introdução: A espessura da cortical óssea vestibular na região anterior da maxila constitui um fator crítico na implantologia, ortodontia e dentisteria estética. A tomografia computorizada de feixe cónico (CBCT), combinada com segmentação baseada em inteligência artificial (IA) e fluxos de trabalho digitais tridimensionais, pode facilitar a avaliação tridimensional da morfologia do osso alveolar. Objetivo: Avaliar a espessura da cortical óssea vestibular na região anterior da maxila utilizando imagens de CBCT associadas à segmentação baseada em IA e à análise digital tridimensional. Materiais e Métodos: Foram analisados retrospetivamente exames de CBCT de pacientes com dentes na região anterior da maxila. Os ficheiros DICOM foram importados para o software CoDiagnostiX® para segmentação baseada em IA e geração de modelos STL. Os modelos obtidos foram analisados no Geomagic Control X® através de um fluxo de trabalho tridimensional padronizado. Foram realizadas medições lineares da espessura da cortical óssea nos terços cervical, médio e apical dos dentes. Foram efetuadas análises estatísticas descritivas, inferenciais e exploratórias para avaliar a distribuição da espessura da cortical óssea e a variabilidade anatómica. Resultados: Foram analisados 44 exames de CBCT. Observou-se uma considerável variabilidade anatómica na espessura da cortical óssea vestibular. Os valores médios de espessura apresentaram, de forma geral, uma diminuição do nível apical para o nível cervical, sendo os menores valores observados no terço cervical. A análise através de modelos lineares mistos demonstrou um efeito estatisticamente significativo do nível radicular na espessura da cortical óssea, tanto no lado direito (p = 0,003) como no lado esquerdo (p = 0,042), verificando-se uma espessura significativamente superior ao nível apical quando comparada com o nível cervical (correção de Bonferroni, p = 0,002). Não foram identificados efeitos estatisticamente significativos da idade ou do sexo. No lado esquerdo, os incisivos laterais apresentaram uma espessura cortical significativamente superior à dos incisivos centrais (p = 0,006). Conclusão: A região anterior da maxila apresenta uma predominância de cortical óssea vestibular fina associada a uma considerável variabilidade anatómica. O presente estudo demonstrou a viabilidade de um fluxo de trabalho digital tridimensional que combina imagens de CBCT, segmentação assistida por IA, geração de modelos STL e análise morfométrica tridimensional para a avaliação da morfologia do osso alveolar. Os resultados do presente estudo poderão contribuir para um planeamento terapêutico mais individualizado em implantologia, ortodontia e dentisteria estética.
