Os seios maxilares, assim como os restantes seios paranasais, ajudam na ressonância da voz, na melhoria do ar inspirado, na diminuição do peso do crânio, no equilíbrio de diferenças de pressão. São ainda uma barreira imunológica, produzindo lisozima.
As comunicações entre a cavidade oral e os seios maxilares são complicações raras de cirurgia oral. A extração de dentes maxilares posteriores é a causa mais comum destas complicações, devido à relação anatómica entre os ápices dos pré-molares e molares e o seio maxilar.
A fístula-oroantral é uma comunicação patológica entre o seio maxilar e a cavidade oral, revestida por epitélio.
A escolha de uma terapêutica adequada ao encerramento da comunicação deve ter em conta: idade, género, raça, largura da fístula, a presença ou ausência de infeções dos seios e patologias locais ou sistémicas.
O acompanhamento médico deve ser mantido até ao encerramento da fístula-oroantral e à eliminação da infeção do seio, para proporcionar boa cicatrização e prevenir recorrências.
Partindo de uma revisão bibliográfica (livros/artigos) pretendo fazer um estudo no âmbito da terapêutica de fístulas oroantrais, ilustrando com casos clínicos de resolução cirúrgica da Clinica Universitária e realizados pelos Docentes de Cirurgia Oral.

The maxillary sinuses, as well as the other paranasal sinuses, help in the resonance of the voice, improve the inspired air, reduce the weight of the skull and balance the differences in pressure. They are also an immunological barrier through the production of lysozyme. Oroantral communications between the oral cavity and the maxillary sinuses, are rare complications of oral surgery. The extraction of posterior maxillary teeth is the most common cause of these complications due to the anatomical relationship between the apexes of the premolars and molars and maxillary sinuses. Oroantral fistulas are pathological connections between the maxillary sinus and the oral cavity, lined by epithelium. The choice of the appropriate therapy should consider age, gender, race, width of the fistula, presence or absence of sinus infections and local or systemic pathologies. The appropriate management should be maintained until the closure of the oroantral fistula and the elimination of the sinus infection to ensure a good healing and to prevent recurrences. Through a review of literature (books and articles) I intend to explore the oroantral fistula treatments, further illustrated using clinical cases from the University clinic, resolved surgically by our Faculty Professors of Oral Surgery.
