Abstract(s)
A realização deste relatório surge em resultado do percurso desenvolvido ao longo do 11º Curso do Mestrado em Enfermagem com Especialização em Enfermagem Médico-Cirúrgica da Escola de Enfermagem da Universidade Católica Portuguesa. Este trabalho incide sobre o estágio desenvolvido numa Unidade de Cuidados Intermédios e numa Unidade de Cuidados Intensivos Polivalente, entre 10 de setembro de 2018 e 05 de janeiro de 2019. O presente relatório assume-se como uma análise crítico reflexiva das competências adquiridas no âmbito da Especialização em Enfermagem Médico-Cirúrgica, fruto das experiências vivenciadas e das atividades desenvolvidas ao longo dos estágios, suportada em pesquisa bibliográfica, com recurso a bases de dados, publicações periódicas e trabalhos de investigação, não descorando a prática. Primeiramente, é realizada uma contextualização dos locais de estágio, provida da respetiva caracterização. De seguida, procede-se à análise crítica reflexiva das atividades realizadas com vista ao alcance dos objetivos delineados no projeto de estágio para a aquisição das competências comuns do enfermeiro especialista, com base nos quatro domínios estabelecidos pela ordem dos enfermeiros. No domínio da responsabilidade profissional, ética e legal, são descritas e analisadas as intervenções relacionadas com o respeito pela liberdade e dignidade da pessoa humana, do dever do sigilo profissional, do direito à informação, assim como do consentimento presumido. No domínio da melhoria contínua da qualidade intervém-se conscientemente e de forma estruturada na procura contínua da melhoria da qualidade e da segurança do doente, família e profissional, visando a excelência do exercício. Foram identificadas oportunidades de melhoria no que se refere à privação do sono e repouso da Pessoa em Situação Crítica internada em Unidades de Cuidados Intensivos, tendo sido elaborada uma proposta de intervenção acerca da importância da sua promoção. Previamente procedeu-se à fundamentação teórica, com recurso a pesquisa bibliográfica que lhe serviu de base. No domínio da gestão de cuidados, realça-se e reflete-se sobre a importância da colaboração com as equipas multidisciplinares, com o intuito de perceber toda a dinâmica e todo o processo de gestão de um serviço. Relativamente ao domínio das aprendizagens profissionais, procede-se à descrição de todas as ações promotoras ao desenvolvimento das competências específicas para a prestação de cuidados à Pessoa em Situação Crítica perante situações complexas e imprevistas, importantes ao autodesenvolvimento e autoconhecimento pessoal e profissional. Na conclusão aborda-se todo o desenvolvimento profissional à luz das competências adquiridas.Como contributo para ambos os contextos de estágio acerca da privação do sono e repouso da Pessoa em Situação Crítica internada, desenvolve-se ações de formação, um protocolo de intervenção e um póster, com o objetivo de sensibilizar as equipas multidisciplinares quanto ao seu valor na recuperação e melhoria do outcome da Pessoa em Situação Crítica. Todas as experiências vivenciadas ao longo do estágio foram alvo de uma reflexão complexa, diferenciada e exigente, o que contribuiu para o desenvolvimento das minhas competências enquanto futura enfermeira especialista em Enfermagem Médico-Cirúrgica e Mestre, permitindo processos de tomada de decisão eficazes e sustentados.
This report was created as a result of the work developed during the 11th Course of the Master's Degree in Nursing with Specialization in Medical-Surgical Nursing of the School of Nursing-Porto of the Universidade Católica Portuguesa. It covers the stages developed in an Intermediate Care Unit and a Multipurpose Intensive Care Unit from september 10, 2018 to january 5, 2019. This report is a critical reflexive analysis of the competences acquired during the course of Specialization in Medical-Surgical Nursing, as a result of the experiences and activities carried out along the stages supported by bibliographic research, using databases, periodicals and research, not detracting from the practice. Firstly a contextualization of the stage sites provided by its characterization. Next, a reflexive critical analysis of the activities was made is order to achieve the goals outlined for the internship aiming the acquisition of the common skills set for the specialist nurse, according to the four domains established by the Order of Nurses. In the field of professional, ethical and legal responsibility, interventions related to respect for the freedom and dignity of the human person, the duty of professional secrecy, the right to information as well as presumed consent are described and analyzed. In the field of continuous quality improvement, there is a conscious and structured intervention seeking the continuous improvement of quality and safety of the patient, family and professional worker, aiming the excellency of the work performed. There was the identification of the opportunities for improvement os sleep deprivation and rest of the Person in Critical Situation in Intensive Care Units, having been done an intervention proposal about the importance of its promotion. Previously, we made a theoretical substantiation resorting to bibliographic research that served as the basis. In the field of care management, the importance of collaborating with multidisciplinary teams is highlighted and reflected in order to understand the whole dynamics and process of managing a service. Regarding the field of professional learning, a description is made of all the actions that promote the development of specific skills to provide care to the critical person concerning complex and unforeseen situations that are important for self-development and personal and professional self-knowledge. In the conclusion, it is approached the professional development according to acquired competences.As a contribution to both stage contexts about the deprivation of sleep and rest of the Person in Critical impatient Situation training actions are developed, an intervention protocol and a poster, to sensitise the multidisciplinary teams as to its value in the recovery and improvement of the outcome of the Person in Critical Situation. In short, all the experiences that were experienced during the internship were subdued to a complex, differentiated and demanding reflection, which contributed to the development of my competences as a future specialist nurse in surgical medical nursing and master in nursing, allowing effective and sustained decision making processes.
