Abstract(s)
O presente relatório surge no âmbito do Mestrado em Enfermagem com especialização em Enfermagem Médico-Cirúrgica, na área de Enfermagem à Pessoa em Situação Crítica. Tem como objetivo descrever e analisar o percurso realizado, recorrendo a metodologia reflexiva, de modo a demonstrar as competências adquiridas e fundamentar a obtenção do grau de mestre em enfermagem e título de enfermeiro especialista, na referida especialidade. Atendendo ao desenvolvimento das competências de mestre descritas no artigo 15º do Regime Jurídico dos Graus e Diplomas do Ensino Superior, foi realizada uma revisão scoping intitulada “O Cuidado Justo em Enfermagem”. Os aspetos identificados neste estudo contribuem para a reflexão sobre o exercício da profissão de enfermagem, tendo também em consideração a conjuntura atual do país e do mundo em que vivemos com a pandemia COVID-19, requerendo um olhar ético que incorpora conceitos centrais para o desenvolvimento de boas práticas de enfermagem, entre os quais se destaca o conceito de justiça, no sentido do cuidado justo. A componente clínica deste percurso académico e profissional realizou-se no Instituto Nacional de Emergência Médica, nomeadamente numa Ambulância de Suporte Imediato de Vida, numa Unidade de Cuidados Intensivos e num Gabinete Coordenador de Colheita e Transplantação. A análise das competências foi realizada com base no Regulamento n.º 140/2019 de 6 de Fevereiro da Ordem dos Enfermeiros e no Regulamento n.º 429/2018 de 16 de Julho da Ordem dos Enfermeiros. No que concerne às competências comuns do enfermeiro especialista destaco o “domínio da responsabilidade ética e legal” onde demonstro competências de tomada de decisão ética e respeito pelos direitos humanos, recolhendo contributos para a análise dos fundamentos das tomadas de decisão e suscitando a reflexão sobre os processos envolvidos. O desenvolvimento das competências específicas do enfermeiro especialista foi transversal nos dois contextos de estágio, sendo que as competências para a prestação de cuidados à pessoa submetida a ventilação mecânica invasiva e a prestação de cuidados à pessoa em morte cerebral potencial dadora de órgãos tiveram um maior destaque. Deste modo, pude desenvolver competências relativas ao cuidado especializado à pessoa e sua família/cuidadores a vivenciar processos médicos e/ou cirúrgicos decorrentes de doença aguda ou crónica nos diferentes contextos, desde a emergência pré-hospitalar até ao internamento numa unidade de cuidados intensivos. Por fim, considero ter atingido os objetivos propostos, tendo desenvolvido competências de enfermeiro especialista que me permitem praticar cuidados de enfermagem especializados nesta área.
This report is part of the Master's Degree in Nursing with a specialization in Medical Surgical Nursing in the field of nursing for people in critical situations. Its purpose is to describe and analyze the course taken, using reflective methodology, so as to demonstrate the skills acquired and support the attainment of the Master's Degree in Nursing and the title of nurse specialist in this specialty. For the development of the master’s degree skills, described in article 15 of the Legal Framework for Degrees and Diplomas in Higher Education, a scoping review entitled "Fair Care in Nursing" was conducted. The aspects identified in this study contribute to the reflection on the exercise of the nursing profession, also taking into account the current situation of the country and the world because of COVID-19 pandemic, requiring an ethical perspective that incorporates key concepts for the development of good nursing practices, among which the concept of justice, in the sense of fair care, stands out. The practical component of this academic and professional journey took place at the National Institute of Medical Emergency, namely in an Immediate Life Support Ambulance, in an Intensive Care Unit, and in an Organ Procurement and Transplantation Coordination Office. The analysis of the competencies was performed based on Regulation no. 140/2019 of February 6 of the Order of Nurses and Regulation no. 429/2018 of July 16 of the Order of Nurses. With regard to the common competencies of the specialist nurse, I highlight the "domain of ethical and legal responsibility" where I demonstrate ethical decision-making skills and respect for human rights, gathering contributions for the analysis of the grounds for decision-making and raising reflection on the processes involved. The development of the specialist nurse's specific skills was transversal in both internship contexts, with a greater emphasis on the skills related to the provision of care to the person undergoing invasive mechanical ventilation and the provision of care to the brain-dead potential organ donor. Thus, I was able to develop skills related to the specialized care of the person and his/her family/caregivers experiencing medical and/or surgical processes resulting from acute or chronic disease in different contexts, from pre¬hospital emergency to hospitalization in an intensive care unit. Finally, I believe that I have achieved the proposed objectives, having developed specialist nursing skills that allow me to practice specialized nursing care in this field.
