O presente relatório tem como objetivo a prestação de prova pública para a obtenção do grau académico de Mestre, comportando uma descrição, análise e reflexão acerca do percurso desenvolvido na unidade curricular de Estágio Final e Relatório do Curso de Mestrado em Enfermagem Médico-Cirúrgica: Enfermagem à Pessoa em Situação Crítica, no Instituto de Ciências da Saúde da Universidade Católica Portuguesa. Para a desejada aquisição e desenvolvimento das competências de Enfermeiro Especialista realizei vários estágios, nomeadamente numa unidade de cuidados intensivos, num serviço de técnicas dialíticas, num serviço de urgência geral e em contexto de emergência extra hospitalar numa viatura médica de emergência e reanimação. A par da aquisição das competências específicas, identifiquei áreas de intervenção que permitiram o meu crescimento enquanto enfermeira, realizando atividades como a dinamização de sessões de reflexão e formação. Na Unidade de Cuidados Intensivos contribuí para a prevenção e controlo da infeção associada aos cuidados de saúde através da colaboração com a minha enfermeira orientadora numa formação sobre a importância da correta higienização das mãos. No serviço de urgência incidi a minha atenção na segurança no transporte da pessoa em situação crítica, demonstrando a importância da utilização da check-list, já existentes no serviço para o melhor planeamento de qualquer transporte. A adaptação dos cuidados de enfermagem à pessoa em situação crítica durante o transporte aéreo foi o fenómeno mais explorado neste percurso, interpretado à luz de dois referenciais teóricos no desenvolvimento do cuidado de excelência: a teoria do conforto de Kolcaba e a teoria de iniciado a perito de Benner. É responsabilidade da enfermagem desenvolver estratégias de adaptação dos cuidados à pessoa no seu ambiente, surgindo a necessidade de identificar as alterações fisiológicas a que está sujeita contornando ao máximo as condições ambientais adversas (Ordem dos Enfermeiros, 2012 a)). Neste contexto de cuidado em transporte aéreo, foi elaborada uma Scoping Review com o objetivo de responder à questão: “Qual a adaptação necessária dos cuidados de enfermagem à pessoa em situação crítica durante o transporte aéreo em aeronave de asa fixa?”.
The purpose of this report is the public examination for the award of the Master's degree, including a description, analysis and reflection on the course developed in the Master's Degree Course in Medical-Surgical Nursing: Nursing in Critical Condition, at the Institute of Health Sciences of the Portuguese Catholic University. For the desired acquisition and development of the competencies of a Nurse Specialist, I carried out several internships, namely in an intensive care unit, a dialysis service, a general emergency service, and in an out-of-hospital emergency context in an emergency and resuscitation medical vehicle. Along with the acquisition of specific skills, I identified areas of intervention that allowed me to grow as a nurse, carrying out activities such as the dynamization of reflection and training sessions. In the Intensive Care Unit, I contributed to the prevention and control of healthcare-associated infections by collaborating with my nurse supervisor in a training session on the importance of proper hand hygiene. In the emergency department, I focused my attention on the safety in the transport of critically ill patients, demonstrating the importance of the use of the checklist already existing in the service for the best planning of any transport. The adaptation of nursing care to critically ill patients during air transport was the most explored theme in this journey, interpreted in the light of two theoretical references in the development of excellence of care: Kolcaba's comfort theory and Benner's beginner to expert theory. Nurses are responsible for developing strategies to adapt care to the person in his/her environment, thus the need to identify the physiological changes to which the person is subjected to adapt, circumventing the adverse environmental conditions as much as possible arises (Ordem dos Enfermeiros, 2012 a)).
