Segundo o Programa da Diabetes (World Health Organization, 2010), a Diabetes é
responsável por 5% das mortes globais anuais. Prevê-se ainda um aumento de mais 50% nos
próximos 10 anos, se não forem implementadas medidas urgentes.
O nosso País não é excepção e para comprovarmos temos os dados recentes do
estudo de Prevalência da Diabetes em Portugal: (PREVADIAB, 2009). Resultados apontam
para 11,7% da população portuguesa ser diabética, o que corresponde a 905 035 indivíduos,
isto é, a um décimo da população portuguesa. Estes dados referem-se à faixa etária
compreendida entre os 20 e os 79 anos.
De acordo com a magnitude destes dados procurou-se desenvolver um diagnóstico de
Saúde à população Diabética da Freguesia de Custóias. Foi caracterizada a população
diabética da Freguesia de Custóias inscrita nas Clínicas Privadas I, II e III com convenção com
o Serviço Nacional de Saúde, no programa de saúde “Grupo de Risco: Diabetes” do Sistema
de Apoio á Prática de Enfermagem (SAPE).
Posteriormente procurou-se desenvolver projectos inseridos num Programa
Educacional para Diabéticos Tipo 2 que de alguma forma procura dar resposta a algumas
necessidades deste programa. O Programa Educacional para Diabéticos Tipo 2 contempla
assim quatro projectos, intitulados: “Pés Saudáveis”, “A Medicação e o Diabético”, “Sessão
Comemorativa do Dia Mundial da Diabetes” e “Vigiar para Controlar”. Estes projectos surgiram
para dar resposta a três prioridades de intervenção na comunidade definidas pelo consenso de
peritos da UCC da Senhora da Hora.
O objectivo foi promover conhecimentos sobre a Diabetes Mellitus nos Diabéticos tipo 2
com convenção com o Serviço Nacional de Saúde, pertencentes á Freguesia de Custóias, que
se encontram inscritos nas Clínicas Privadas I, II e III.
No desenvolver do projecto foram estabelecidas parcerias com diferentes entidades,
entre elas, a Junta de Freguesia da Senhora da Hora, a Junta de Freguesia de Custóias, o
Centro de Saúde da Senhora da Hora e a UCC da Senhora da Hora. Esta articulação foi
fundamental para a actuação do enfermeiro como Especialista em Enfermagem Comunitária.
As estratégias adoptadas foram:
- Curso de Formação para os Diabéticos Tipo 2 com convenção com o Serviço
Nacional de Saúde, pertencentes à Freguesia de Custóias, inscritos nas Clínicas Privadas I, II
e III, sobre Pé Diabético, Auto-vigilância da Glicemia Capilar e Regime Medicamentoso.
- Coordenar a Sessão Comemorativa do Dia Mundial da Diabetes na Freguesia de
Custóias;
- Visitas Domiciliárias aos diabéticos dependentes em grau moderado a elevado que
não puderam participar do Curso;
- Formação em serviço para todos os Profissionais de Saúde do Centro de Saúde da
Sra. da Hora.
According to the Diabetes Program (World Health Organization, 2010), diabetes accounts for 5% of global deaths annually. It is also expected an increase of over 50% in the next 10 years if urgent measures are not implemented. Our country is no exception and to show that, we have the recent data from the study of Diabetes Prevalence in Portugal: (PREVADIAB, 2009). Results indicate 11.7% of its population have diabetes, corresponding to 905 035 individuals, one third of its population. These data refer to the ages between 20 and 79 years. According to the magnitude of these data we sought to develop a health diagnosis to the Diabetic Population of the Parish Council of Custóias. We characterized the Diabetic Population of the Parish Council of Custóias that were registered on the private clinics I, II, III with convention with the National Health Service, on the Health Program “Risk Group: Diabetes” of the Support System of the Nurse Practice (SAPE). After that we tried to develop projects included in an Education Program for Type 2 Diabetics that somehow seeks to address some needs of this program. The Education Program for Type 2 diabetes includes four projects, entitled: “Healthy Feet”, “Medication and Diabetes”, “Commemorative Session of the World Diabetes Day” and “Watch to Control”, These projects have emerged to address these priorities for action in the community defined by the consensus from the UCC of Senhora da Hora. The aim was to promote knowledge about diabetes mellitus type 2 on diabetics, in agreement with the National Health Service, belonging to Custóias, who are registered in Private Clinics I, II and III. During the developing of the project, were established partnerships with various entities, including Parish council of Senhora da Hora, Parish council of Custóias and the Health Center and UCC from Senhora da Hora. The joint action was essential for the nurse conduct as a Master in Community Nursing. The strategies adopted were: - Training Course for Diabetics Type 2, in agreement with the National Health Service, belonging to the Town of Custóias, enrolled in Private Clinics I, II and III on the Diabetic Foot, Self-monitoring of blood glucose monitoring and medication regime. - Coordinate the Commemorative Session of the World Diabetes Day in the Town of Custóias; - Home visits for diabetics dependent on moderate to high that could not attend the course; - In-service training for all Health Care Professionals of Sra. da Hora Health Center
