(Re)Descobrir a Comunicação no Cuidar é um relatório que compila as aprendizagens
inerentes ao meu desenvolvimento de competências de enfermeira especialista em
enfermagem médico-cirúrgica. O seu objetivo coaduna-se com o descrever de forma reflexiva,
crítica e fundamentada, o desenvolvimento das atividades e competências ao longo dos
estágios. Integrei uma metodologia descritiva, analítica e reflexiva, apoiado em recursos
bibliográficos de bases de dados científicas, motores de busca ou pesquisas manuais em
bibliotecas. A comunicação enfermeiro-pessoa foi o foco de enfermagem condutor e
transversal ao longo dos três estágios que realizei e que explorei pela ordem cronológica de
concretização: Cuidados Intensivos, Opcional (Paliativos) e Serviço de Urgência. Como suporte
a este percurso seguiram-se as orientações teóricas de Ida Orlando e Jean Watson, numa
simbiose de complementaridade entre diferentes paradigmas de enfermagem.
No estágio de Cuidados Intensivos destaco a minha integração no Projeto PQCE/CIPE do
CHLC, no qual colaborei formando a equipa de enfermagem da Unidade, com a partilha da
minha experiência profissional. Realço também a revisão sistemática da literatura, que se
encontra em fase de análise por uma revista científica de enfermagem, e que serviu de
sustentação na partilha dos resultados com a equipa de enfermagem e na elaboração de um
“Quadro de Comunicação” como instrumento facilitador da comunicação não verbal na pessoa.
Intrínseco à especificidade dos Cuidados Paliativos, direcionei o Módulo Opcional para a
vertente psicossocial do cuidar. Desenvolvi a capacidade de relação de ajuda e comunicação,
trabalhei a esperança na pessoa em fim de vida e transferi formalmente os conhecimentos
adquiridos em terapêutica subcutânea e pessoa com delirium para o meu local de trabalho,
com grande envolvimento da minha equipa de enfermagem. Revi o “Protocolo de Atuação no
Delirium” do serviço de cuidados paliativos. Apresentei também uma Comunicação Poster num
evento científico com o tema “Comunicar em Cuidados Paliativos”.
No que se refere ao estágio em Serviço de Urgência realço as situações novas e imprevistas
que despoletaram desafios ao cuidar no imediato. Na melhoria contínua dos cuidados partilhei
com os chefes de equipa uma reflexão acerca da passagem de turno, que compila
simultaneamente práticas e evidência científica. No sentido da partilha de conhecimentos
adquiridos acerca da eletrocardiografia e cuidados inerentes, realizei uma sessão de formação
no meu local de trabalho. Na esfera da comunicação, criei um instrumento de trabalho que
denominei “Roda da Comunicação”, onde disponibilizei orientações práticas aos enfermeiros e
que foi apresentado em sessão de formação. Em curso está a realização de um estudo de
caso que recentemente obteve parecer positivo da Comissão de Ética para a Saúde da
Instituição.
Este é um percurso que não termina aqui, mas continuar em cada momento da minha prática
profissional, contagiando os meus pares para o desabrochar da capacidade de cuidar com um
novo olhar, o olhar da Enfermagem Avançada.
(Re)Discovering Communication in Caring is a report that compiles the learning in the development of my skills as a specialist nurse in medical-surgical nursing. Its purpose is describing in the reflective, critical and reasoned way the activities and skills developed throughout the stages. I integrated a descriptive, analytical and reflective methodology, supported by bibliographic resources of scientific databases, search engines and manual searches in libraries. The nurse-person communication was the focus of nursing that cross over the three stages that I have explored in the chronological order: Critical Care, Optional (Palliative Care) and Emergency Room. To support this route I followed the theoretical orientations of Ida Orlando and Jean Watson, into a symbiosis of complementary between different paradigms of nursing. In stage Intensive Care I highlight my integration into PQCE / CIPE's CHLC Project, in which I collaborated with the formative part in the unit nursing team by sharing my experience. In this module, I highlight the systematic literature review under analysis for publication in a nursing scientific journal, which served as support in sharing the results with the nursing staff and the preparation of a “Communication Framework” as facilitator of nonverbal communication in person. Intrinsic to the specificity of Palliative Care, I directed the Optional Module for the psychosocial aspects of care. I developed the ability to help relationship and communication, worked hope in the person in the end of life and formally transfered to my workplace the knowledge gained in subcutaneous therapy and in person with delirium, with great involvement of my nursing staff. I reviewed the "Protocol Performance in Delirium" of palliative care service. I have also presented a Poster Communication on a scientific event under the theme "Communication in Palliative Care." In the Emergency Room stage I highlight the vast experience of new situations and unforeseen that triggered challenges in the care. For continuous improvement of care I shared with team leaders a reflection about the shift report, which compiles both practice and scientific evidence. In order to share the knowledge gained on electrocardiography and on inherent care, I have done a training session at my workplace. In the sphere of communication, I created a working tool which I called "Wheel of Communication", which features practical guidance to nurses and has been presented in the training session. Ongoing is a case study that I have already a positive decision from the Ethics Committee for Health of the Institution. This is a journey that does not end here, but I have the ambition to continue at every moment of my professional practice, infecting my peers for the blossoming of the ability to care with a new look, the look of Advanced Nursing.
