  • Comunicar com a pessoa submetida a ventilação mecânica invasiva: que estratégias? - revisão sistemática

    Objetivo: conhecer as estratégias de comunicação com a pessoa submetida a ventilação mecânica invasiva. Metodologia: revisão sistemática com o propósito de responder à questão «Comunicar com a pessoa submetida a ventilação mecânica invasiva: que estratégias?», por meio de estudos empíricos, em bases de dados e bibliotecas virtuais, entre janeiro de 2001 e dezembro de 2011, selecionados de acordo com o método PICOS. Resultados: as estratégias devem ser criteriosamente selecionadas e adaptadas às necessidades da pessoa em situação crítica; as estratégias mais simples e acessíveis assumem-se como preferenciais, podendo ser utilizadas de modo isolado ou combinado. Conclusão: as estratégias para comunicar com a pessoa submetida a ventilação mecânica invasiva são sinais sim-não, VOCAs (Voice Output Communication Aids), gestos, acenos de cabeça, leitura de lábios, aperto de mãos, expressão facial, papel e caneta, uso do alfabeto, quadro de palavras e imagens, linguagem corporal e toque.
  • It diminishes the risk factors, prevents the diabetes type 2

    Objective: to describe the risk factors that contributes to the onset of type 2 diabetes. Method: to perform this article we proceeded to an integrative review of Portuguese and international literature between the years 2000 and 2011 held in the database of the Scientific Open Access Repository of Portugal and Medline, with the descriptors for type 2 diabetes mellitus, risk factors, nursing in Portuguese and English. Results: the onset of type 2 diabetes is a combination of genetic, environmental and behavioral factors, associated with lifestyles, characterized by inactivity, and the proportional increase in caloric intake. Conclusion: you can change the natural history of type 2 Diabetes Mellitus, preventing it from occurring by practicing healthy behaviors. The nurse should create projects for the prevention of type 2 diabetes, including strategies and activities to reduce risk factors and working together with citizens, contributing to a clarification, training, and thus improving health levels
  • Avaliação do risco de diabetes tipo 2 nos cuidados de saúde primários

    Objetivos: conhecer os fatores de risco para a diabetes tipo 2 dos utentes com 45 e mais anos e estruturar um projeto orientado para a prevenção da diabetes mellitus tipo 2. Método: estudo descritivo e correlacional, de abordagem quantitativa. Amostra foi de 200 utente, inscritos na Unidade Funcional de Loulé, Portugal. Este estudo foi realizado de acordo com a declaração de Helsinque, que estabelece as normas fundamentais para a ética em pesquisa internacional. Resultados: foram detectados 49 indivíduos com alto risco e 4 indivíduos com risco muito alto de desenvolverem a diabetes tipo 2. Foram diagnosticados 4 novos casos de pessoas diabéticas. Conclusão: desenhou-se o projeto “Diminua o Risco, Previna a Diabetes Tipo 2”, de modo a diminuir os fatores de risco e aumentar os ganhos em saúde.
  • Validación del índice de bien-estar personal en personas con enfermedad renal crónica

    Objective: to verify the psychometric properties of the Personal Wellbeing Index (PWI) in people with chronic kidney disease on hemodialysis. Method: This is a methodological study. A random sample consisted of 171 people with Chronic Kidney Disease (CKD) who underwent hemodialysis in two clinics in the region of Lisbon, Portugal. Data collection was conducted from May to June 2015. The psychometric properties were evaluated: Validity (construct, convergent predictive and discriminative), reliability through internal consistency (Cronbach α) and stability (Spearman-Brown correlation coefficient and intraclass correlation coefficient (ICC)). Results: PWI has a dimension adjusted to the data set. Satisfaction with health is the largest contributor to the PWI. It shows convergent validity with Subjective Happiness and Positive Affect and Negative Affect. It has discriminative power of nationality. The Cronbach α was 0,82. Retest test for the questionnai re 0, 92 and ICC = 0,95, for the interview 0,85 and ICC = 0,89. Conclusions: The Portuguese version of PWI is valid and reproducible in people with CKD, both by questionnaire and by interview.
  • Intervenção comunitária em pessoas idosas com obesidade

    Objetivo: contribuir para a diminuição da prevalência da obesidade nas pessoas idosas, numa freguesia do concelho de Beja, Sul de Portugal. Metodologia: projeto de intervenção comunitária, com duração de 12 meses. A antropometria é recolhida em três pontos-chave: pré-intervenção, seis meses depois e no final do projeto, determinando Indice de Massa Corporal e Perímetro Abdominal, e negociando um contrato de saúde, em consulta de enfermagem. O acompanhamento foi efetuado às 4, 12 e 20 semanas, por entrevista direta aos participantes. Resultados: 60% dos utentes referiram ter modificado alguma prática alimentar; 17.7 % iniciaram e mantêm atividade física regular e em 8,8 % mantêm-se o consumo de água recomendado. Conclusão: os participantes colaboraram nos aspetos relacionados com as alterações na dieta mostrando dificuldades na prática de atividade física regular e no consumo de água.
  • Hypertension and adherence to the therapeutic regímen in primary health care

    Objective: empowering a person with high blood pressure for therapeutic adherence. Method: we conducted a descriptive study, cross-sectional with quantitative approach to diagnosis of the behaviors of adherence to therapeutic regimen in the Community of Comporta – Portugal. The sample was 61 hypertensive people, over 18 years old, who perform drug regimen, which also attend medical appointment of high blood pressure in health center and who agreed to participate by signing an informed consent. The questionnaire was the data collection instrument. Results: the studied population is as majority of female gender, married, low education, retired. The median is between 60-70 years old and most of them live with relatives. The dietary habits must change the level of fats and salt consumption. The analysis allows us to observe that medication adherence is high, but only 6% of the participants practice physical exercise and half of these walks and does hydro gymnastics. The results support the need to design community projects promoting health, and also the behaviors and lifestyles seems very important in the adherence. Conclusion: Therapeutic adherence is a focus of attention of nurses, covering aspects ranging from the organization of nursing appointment and outline minimum nursing data to develop strategies to ensure treatment adherence. We drew up the project "Intervening to Prevent" with the purpose of enabling the person with high blood pressure for adherence to treatment regimen, with involvement in education and therapeutic physical exercise programs.
  • Validation of the positive and negative affect schedule in people with chronic kidney disease

    Aim: to analyze the psychometric properties of the Portuguese version of the Positive and Negative Affect Schedule scale in people with chronic kidney disease on hemodialysis. Method: this was a methodological study. A random sample of 171 people chronic kidney disease who receive hemodialysis in two clinics in the region of Lisbon, Portugal. Data was collected from May to June 2015. The psychometric properties were evaluated: Validity (construct, convergent and predictive), reliability (Cronbach’s α) and stability (test-retest). Results: showed that the Positive and Negative Affect Schedule scale has two distinct dimensions of positive and negative affect, reliable and stable both when obtained by interview or by questionnaire. Conclusion: the validity and reproducibility of the Portuguese version of the Positive and Negative Affect Schedule scale in people with chronic kidney disease are consistent with the original version and the English version of this scale.
  • Community health intervention project active ageing-ripen into health

    Objective: to implement the intervention project: Active Ageing – Ripen into Health, based upon a partnership of the Unidade de Cuidados na Comunidade (UCC) / Universidade Sénior. Method: methodology of the project, based on the ®Work Breakdown Structure. Documentary research was used. The data collection was carried out with the target population and stakeholders. The target population was constituted by the 23 students signed in the project. Two questionnaires were applied to them (The Method of Biopsychosocial Evaluation, from the Portuguese National Network for Integrated Continuous Care - Rede Nacional de Cuidados Continuados Integrados (RNCCI) and another one with the purpose to identify the suitable education needs for health); a questionnaire was made to the stakeholders (Identification of the suitable education needs for health) as well an interview was performed. The collected data have been subjected to statistical treatment and content analysis. Results: the analysis of the results obtained, allowed to structure an Intervention Programme based upon the National (Portugal) guidelines for the health of elderly people, suitable for the targeted, the available resources as well our purposes and final objective. Conclusion: The objectives were reached: the partnership proposed was implemented and the Intervention Programme is being carried out. The project promoted the reflexion and the work of the multidisciplinary and inter-sectoral team in the community; at the same time, it pointed out to the attention for the individual and collective responsibilities in response to problems related to active ageing. Viewing that the nurses of community health are key elements on the group intervention in all the stages of the life cycle, either due to their multi-sectoral framework or by all the knowledge they acquired, this way, the key factors were converged to become it a successful project, so, actually contributing for gains in health, based on the healthy aging.
