- Development of a form to assess the self-care competence of the person with a tracheostomyPublication . Queirós, Sílvia Maria Moreira; Santos, Célia Samarina Vilaça de Brito; Brito, Maria Alice Correia de; Pinto, Igor Emanuel SoaresBackground: The development of the self-care competence in the person with a tracheostomy is described as a key driver for a healthy transition to life with a tracheostomy. An instrument for assessing the self-care competence of the person with a tracheostomy contributes to nurses’ decision-making and promotes the quality of care. Objectives: To develop a tool for assessing the self-care competence of the person with a tracheostomy. Methodology: A methodological study was conducted. Content validity was obtained through the judgment of experts, with a focus group methodology. Reliability was assessed using Cronbach’s alpha coefficient. The instrument was applied to a sample of 80 participants, using a non-probability convenience sampling technique. Results: The study resulted in a form with 39 indicators, divided into 6 areas, which showed good internal consistency (alpha = 0.89) and recognised content validity. Conclusion: The final version of the form is applicable, and has the potential to be used in future studies on its validity.

Pinto, Igor Emanuel Soares; Queirós, Sílvia Maria Moreira; Queirós, Cármen Dolores Ribeiro; Silva, Carla Regina Rodrigues da; Santos, Célia Samarina Vilaça de Brito; Brito, Maria Alice Correia deEnquadramento: A confeção de um estoma constitui-se como um evento gerador de mudanças influenciado por diversos fatores, nomeadamente a presença de complicações do estoma e/ou pele periestomal. Estima-se que 80% das pessoas com ostomia experienciam, pelo menos, uma complicação relacionada com o estoma ao longo da sua vida. Objetivos: Identificar os fatores de risco associados ao desenvolvimento de complicações do estoma de eliminação e da pele periestomal. Metodologia: Revisão da literatura, com base na estratégia metodológica do Instituto Joanna Briggs para Scoping Reviews. Foram identificados 1492 artigos, sendo incluídos para análise 22.Resultados: A maioria dos fatores de risco para o desenvolvimento de complicações não é modificável. Dos sensíveis à intervenção do enfermeiro evidenciam-se a educação pré e pós-operatória, a marcação do local do estoma e o acompanhamento após a alta hospitalar. Conclusão: O reconhecimento de fatores de risco associados ao desenvolvimento de complicações do estoma permite ao enfermeiro, por um lado, identificar precocemente indicadores de vulnerabilidade nos seus clientes e, por outro lado, intervir de forma mais efetiva.
- Patients’ perspectives on promoting tracheostoma self-care: a qualitative studyPublication . Queirós, Sílvia Maria Moreira; Pinto, Igor Emanuel Soares; Santos, Célia Samarina Vilaça Brito; Brito, Maria Alice CorreiaObjective: To explore patients’ perspective about promoting tracheostoma self-care. Background: Promoting self-care is one of the main areas of intervention for nurses who care for patients undergoing tracheotomy/total laryngectomy. Self-care can improve autonomy, quality of life and reduce tracheostoma complications. To achieve this, interventions need to be evidence-based and sensitive to the patients’ needs and preferences. However, little is known about patients’ experiences of learning to care for their tracheostoma and their preferences. Study design and methods: A qualitative descriptive study was carried out through semi-structured, individual, and face-to-face interviews with patients who had undergone tracheotomy/ total laryngectomy (n = 17). Purposive sampling was performed with maximum variation. All interviews were audio-recorded and transcribed verbatim, and data were analysed using qualitative content analysis. The study complied with the Consolidated Criteria for Reporting Qualitative Research (COREQ). Results: This study identified two main themes. The first was the experience of living with a tracheostoma. Participants recognised the implications of the tracheostoma in everyday life, the importance of tracheostoma awareness, critical moments, possible response patterns, and the personal, social, and health-related factors that can affect this experience. The second theme identified was the promotion of tracheostoma self-care. The participants mentioned the rebuilding of autonomy as the main objective, the thematic content that should be addressed by nurses before and after the surgery, the different strategies and resources that can be used, and what determines the appropriate time to intervene. Conclusion: Despite the challenges of living with a tracheostoma, participants acknowledged the promotion of self-care as crucial for regaining autonomy. Nursing interventions that incorporate contents, methodologies, and implementation moments sensitive to the preferences, needs, and characteristics of each patient, which are maintained over time, appear to be the desired approach according to the participants. Implications for research, policy, and practice: This study’s findings can help to design and implement nursing interventions to promote tracheostoma self-care that leads to better health outcomes. What is already known about the topic? • Undergoing a tracheotomy/total laryngectomy requires adapting to a new condition of life. Promoting self-care in these patients enhance autonomy, and adaptation to the tracheostoma. • Nursing care for tracheostoma patients is poorly systematised. So, interventions to promote tracheostoma self-care should be created and tested, to improve clinical practice. • Nursing interventions tailored to the specificities, needs, and preferences of the target population are more likely to be accepted and effective. Therefore, it is important to understand patients’ perception and involve them in identifying appropriate interventions. What this paper adds • This study can help nurses to understand the complexities faced by people living with a tracheostoma. Recognising the implications, critical events, response patterns, and factors that influence this experience can assist nurses in anticipating care needs over time. • The study participants emphasised the relevance of self-care for maintaining autonomy. They identified the issues, strategies and resources, and significant moments for promoting self-care for people with a tracheostoma. • The results of this study can contribute to defining more coordinated, timely, and tailored nursing care models that meet the needs of individuals and improve health outcomes.
- Promotion of tracheostomy self-care: a qualitative study based on the nurses’ perspectivePublication . There is little information on the nurses’ perspective as care providers with knowledge of clinical contexts. Design: A qualitative descriptive study was conducted. Methods: A purposeful sample of nurses was selected from eight Portuguese hospitals. Recruitment ended once data saturation was reached (n = 12). Data were collected using semi-structured interviews and analysed through content analysis. Results: The nurses described the most important aspects to promote tracheostomy self-care, namely the objectives, the contents to be addressed, the methods and resources to be used, the appropriate moment to intervene, the necessary conditions to promote self-care, the way to ensure the standardisation of interventions among nurses, and the level of tailored planned interventions. Conclusion: From the nurses’ perspective, promoting tracheostomy self-care is critical for regaining autonomy. Therefore, it is crucial to establish an intervention plan sensitive to individual needs and preferences. These results can help elucidate the aspects considered by nurses when promoting tracheostomy self-care so that more meaningful and feasible care models can be designed.
- Psychometric properties of the form development of self-care competence of the person with a bowel elimination ostomyPublication . Pinto, Igor Emanuel Soares; Santos, Célia Samarina Vilaça de Brito; Brito, Maria Alice Correia de; Queirós, Sílvia Maria MoreiraBackground: The use of an instrument to assess the self-care competence in patients with bowel elimination ostomy allows for the systematic identification of their specific needs, ensuring the continuity of nursing care. Although an instrument that assesses the domains that integrate this competence has been developed in Portugal, it has not yet been validated. Objectives: To assess the psychometric properties of the form Development of Self-Care Competence of the Person with a Bowel Elimination Ostomy. Methodology: Methodological study aimed at analyzing the psychometric properties of a measurement instrument, written in Portuguese. Results: The Cronbach’s alpha was 0.95 and the inter-rater reliability showed a correlation greater than or equal to 0.98. A statistically significant correlation was found in the split-half test, as well as in the clinical sensitivity analysis. Conclusion: The form presented good psychometric indicators, which suggests that it is a useful tool to be used in the nursing diagnosis process, thus contributing to the optimization of nursing care.
- A transição para a vida com estoma de eliminação intestinal : um programa de intervenção de enfermagemPublication . Pinto, Igor Emanuel Soares; Brito, Maria Alice Correia de; Santos, Célia Samarina Vilaça de BritoA pessoa com um estoma de eliminação intestinal lida diariamente com desafios físicos, psicológicos e sociais. A forma como este evento é vivenciado é influenciada por diversos fatores, sendo o aspeto com maior impacto na adaptação a competência de autocuidado ao estoma. Os programas de intervenção de enfermagem existentes, tanto a nível nacional como internacional, focam-se na adaptação ou na enunciação semântica das intervenções, por vezes limitando-se a apenas alguns momentos do perioperatório ou a alguns domínios da competência de autocuidado. Para além disso, não definem de forma objetiva e rigorosa as intervenções de enfermagem, os seus conteúdos, métodos de administração e a dose. Assim, o objetivo deste projeto foi construir e avaliar a adequação e a viabilidade de um programa de intervenção de enfermagem para a promoção do autocuidado na pessoa com estoma de eliminação intestinal, servindo como um guia para os enfermeiros na prática clínica. Para alcançar os objetivos propostos, foi utilizada a metodologia do Medical Resourch Council para o desenvolvimento de intervenções complexas. Inicialmente foi realizada uma scoping review da literatura, centrada nas intervenções direcionadas para a promoção do autocuidado ao estoma de eliminação intestinal. De seguida, foi realizado um estudo qualitativo com recurso a entrevistas semiestruturadas e análise de conteúdo segundo Bardin com análise categorial, com o objetivo de conhecer a perspetiva dos enfermeiros da prática clínica e dos clientes sobre a promoção do autocuidado na pessoa com estoma de eliminação intestinal. Com base na evidência identificada e nos resultados do estudo qualitativo, foi desenhada uma primeira versão do programa de intervenção de enfermagem para a promoção do autocuidado ao estoma de eliminação intestinal (PIPOEI). Para avaliar a adequação e viabilidade do programa foi realizado um estudo qualitativo por grupo focal, com enfermeiros de cuidados em estomaterapia e com professores da área do ensino de enfermagem no âmbito do autocuidado e da estomaterapia. Os conteúdos e estrutura propostos para o PIPO-EI foram avaliados como representativos das necessidades e aplicáveis em contexto clínico. Adicionalmente, foram desenvolvidos e avaliados cinco panfletos informativos direcionados para o autocuidado ao estoma que se mostraram relevantes e com potencial para serem um recurso útil no apoio à intervenção do enfermeiro na promoção do autocuidado ao estoma de eliminação intestinal em Portugal. Os resultados desta tese contribuem para a disciplina de Enfermagem, podendo melhorar a qualidade e equidade dos cuidados à pessoa com estoma, apoiando no planeamento e emergindo como suporte para os enfermeiros.
