- The CHANT's conceptual and psychometric validity in Switzerland: a descriptive three-round multicentre e-Delphi studyPublication . Knowing their levels of ecoliteracy would help to make them more effective. Background/Objectives: This descriptive study will use a three-round, multicentre, modified e-Delphi survey to establish an expert panel’s consensus on the Climate, Health, and Nursing Tool’s (CHANT) item-level and scale-level content validity indices. It will also examine potential associations between the expert panel members’ sociodemographic and professional characteristics and their content validity index assessments of the CHANT. Methods: The study will be conducted in the French-speaking regions of Switzerland, running its three-round e-Delphi survey between January and April 2025. After each round, the CHANT’s overall scale-level and individual item-level content validity indices will be computed. Comparisons between different types of healthcare professionals’ profiles will also be conducted. Results: The three-round modified e-Delphi survey should allow the expert panel to reach a consensus on the CHANT’s overall content validity index. The tool should then be considered suitable for pilot testing. The first round brought together 16 experts from different regions, namely French-speaking Switzerland, France, and Belgium. Conclusions: To ensure that the nursing discipline is well positioned to meet future challenges, the development of ecoliteracy must be integrated into nursing education. Ensuring the CHANT’s conceptual and psychometric validity will be essential in strengthening nursing competencies in and knowledge about planetary health and in implementing future educational interventions.
- Chronic wound assessment: cultural and linguistic adaptation for European Portuguese of RESVECH-2 scalePublication . This procedure allows clinicians to verify the appropriateness of their activities and whether the healing process is evolving as expected. Aim: To conduct a cross-cultural adaptation and psychometric analysis of Portuguese version of RESVECH 2.0. Methods: A quantitative and correlational study was designed and, to perform the cross-cultural adaptation of RESVECH 2.0, we followed the classic sequential approach for linguistic equivalence to European Portuguese. The study occurred at a Portuguese oncology hospital and the sample encompassed 281 patients with multiple chronic wounds. Results: RESVECH 2.0 is a practical measurement instrument, easy to use, and well accepted by nurses to know all kinds of wounds’ etiologies. The reliability test revealed an acceptable internal consistency and high proportion of agreement between two raters assessing the same patient. Construct validity was considered average/good and the principal component factor analysis with varimax rotation obtained six factors corresponding to 59.5% of explained variance. When comparing the domains from RESVECH 2.0 with those from BWAT we found statistically significant correlations. Conclusion: The adapted version of RESVECH 2.0 scale presents a good internal consistency and is valid for the Portuguese language and culture, being useful and effective in clinical practice.
- Desafios no diagnóstico diferencial nas lesões por pressão – relato de um caso clínicoPublication . Método: Caso clínico referente a um doente do sexo feminino, com 65 anos de idade, portador de 2 lesões na região perianal, com área de 24cm2 cada uma. Resultado: Após 4 semanas de tratamento com ensyme alginogel®, as lesões apresentaram cicatrização completa. Conclusão: Considerando as Lesões por Pressão um evento adverso, sem dúvida, constituem um desafio para os profissionais e instituições de saúde. O diagnóstico diferencial é essencial para a decisão no tratamento e na prevenção de futuras lesões.
- Educational technology on urinary incontinence during pregnancy: development and validation of an online course for the Brazilian populationPublication . Materials and methods: This methodological study followed an online course’s literature search, development, and validation steps. A total of 22 specialists participated in the validation step, and the content validity index (CVI) was used. Fifty-one Physical Therapy students (target audience) also participated in the Suitability Assessment of Materials. Results: The synthesis reached in the integrative review provided the basis for the course’s theoretical content, which was regarded as suitable by the specialists regarding its con-tent, language, presentation, stimulation/motivation, and cultural adequacy (CVI = 0.99). The target audience considered the course organized, easily understandable, engaging, and motivational, with a positive response index ranging from 84.3 % to 100 %. Conclu-sions: The Brazilian version of the online course was considered sufficiently adequate in content and interface quality by both specialists and the target audience.
- Effect of pressure loading on heel skin temperature and moisture in healthy volunteers: preliminary resultsPublication . The objective was to evaluate the effect of pressure loading on skin temperature and moisture in the heels of healthy adults. This is a before-and-after study performed at Brazil, in October 2022. Skin temperature (◦C) was measured by an infrared digital thermometer, and skin moisture (%) using electrical bioimpedance. Ten individuals/twenty heels were evaluated. The average temperature of the right and left heel was the same at baseline (27.2 ◦C). It was recognized that after 30 min of pressure loading on the heels, there was a decrease in temperature, and after 15 min of pressure offloading, the temperature decreased again. It was found that at t0, the moisture of the right heel (12.6%) was lower than the left heel (15.6%). The median moisture in the right heel increased from t0 to t1 and decreased in t2, while in the left heel, there was a small variation of the median from t0 to t1, as well as to t2. The pressure loading leads to a decrease in temperature and changes the skin moisture of the heels of healthy individuals.
- Exploring climate change’s impact on the cardiopulmonary health of adults living in the Canton of Valais, SwitzerlandPublication . In 2016, Swiss emergency departments (EDs) treated 1,722,000 cases, with 4718 daily admissions. In 2023, the ED of Sion Regional Hospital recorded 75,000 consultations. The links between climate change and health are complex, necessitating urgent research on its impact on cardiopulmonary health in Valais, Switzerland. Raising awareness among frontline professionals is crucial for developing health promotion and disease prevention strategies. Objective: This study explores the preliminary effects of climate change on cardiopulmonary health in Valais and assesses adult patients’ knowledge of its health consequences. Findings will inform adaptations in patient care, health promotion, and disease prevention at Sion Hospital’s ED. The feasibility of patient selection and data collection will also be evaluated. Methods: Using a convergent, parallel, mixed methods design, data will be collected from September 21, 2024, to September 20, 2025, with a target sample of 60 patients. The quantitative phase will examine patient recruitment feasibility, consultation reasons, and triage levels, correlating them with climate variables (temperature, nitrogen dioxide, particulate matter, sulfur dioxide, and ozone). It will also analyze sociodemographic profiles. The qualitative phase will explore patients’ knowledge of climate change and its potential links to their ED visits. The feasibility and acceptability of the study process will be assessed. The protocol follows the SPIRIT (Standard Protocol Items: Recommendations for Interventional Trials) Extension for Pilot and Feasibility Trials. Results: Data collection started on September 21, 2024, following the approval by the ethical commission. Data collection will take place over 1 year, until September 20, 2025. Conclusions: This study will test the feasibility of a larger investigation and examine potential associations between Valais’ changing microclimate and population health. Findings will establish patient profiles and explore their perceptions and knowledge of climate change, informing future health interventions.
- Feridas : prevalência e custosPublication . Alves, Paulo Jorge Pereira; Vieira, Margarida; Amado, João CostaINTRODUÇÃO: A prevalência de feridas nas unidades de saúde Portuguesas não é totalmente conhecida; simultaneamente a gestão de custos do seu tratamento é insuficiente. Para caraterizar este fenómeno decidimos estudar o portador de feridas, a epidemiologia e custos associados ao seu tratamento, nos contextos de cuidados de saúde primários (CSP) e diferenciados (CSD), organizados por NUTS II. MATERIAL E MÉTODOS: Estudo epidemiológico que decorreu no período de Janeiro de 2012 a Dezembro de 2013, tendo sido estudados todos os utentes internados nos CSD e os que recorreram aos CSP. As feridas foram agrupadas de acordo com a etiologia e cronicidade; bem como, caracterizadas segundo a localização, origem, tempo de existência, dor, pele circundante e infecção. Foram, ainda, estimados os custos diretos do tratamento, por tipologia e contexto de cuidados. RESULTADOS: Dos 108840 utentes examinados, 5274 apresentavam feridas, correspondendo a uma prevalência de feridas de 4,84% nos utentes observados e uma prevalência estimada de 3,3 portadores de ferida por 1000 habitantes em Portugal. A prevalência global de feridas nos CSD foi de 33%, onde 1 em cada 3 utentes internados apresenta ferida, e nos CSP foi de 2,2 /1000 habitantes. As feridas crónicas representam uma prevalência estimada de 1,6 /1000 habitantes: Úlcera Pressão 0,7/1000 habitantes; Úlcera Perna 0,7/1000 habitantes; Pé Diabético 0,15/1000 habitantes; e Ferida Maligna 0,1/1000 habitantes. Quanto ao portador de ferida, dos fatores de risco mais associados salientam-se a HTA (40%) e a diabetes (23%). Foram identificadas: taxa de infeção nos tecidos profundos em 17,6% e dor na ferida em 65,7%; A dimensão média da ferida foi de 15,39 cm2, superior da ferida crónica em relação ferida aguda (37,81cm2 Versus 15,39 cm2). O tempo de existência de uma ferida foi em média de 189 dias, sendo significativamente superior na ferida crónica em relação à ferida aguda (412 dias versus 32 dias) . A duração média por tratamento foi de 15,72 minutos, com um custo médio estimado em 8,12€, sendo que o valor médio foi significativamente superior em nas feridas crónicas em relação às feridas agudas (11,01€ versus 6,08€). CONCLUSÃO: Os resultados deste estudo constituem-se num recurso valioso para a gestão estratégica dentro do Sistema Nacional de Saúde em termos de requisitos educacionais, diretrizes clínicas, custos e iniciativas de investigação.
- Feridas complexas: abordagem por equipa multidisciplinar. Uma scoping reviewPublication . Miranda, Liliana Sofia Grilo; Alves, Paulo Jorge Pereira; Amado, João Daniel NevesObjetivos: - reconhecer a importância da equipa multidisciplinar na abordagem à pessoa com ferida complexa na cicatrização de feridas; - mapear a evidência/conhecimento sobre a temática em estudo. Método: Scoping review realizada entre 04 de janeiro e 01 de fevereiro de 2020, nas bases de dados Repositório Científico de Acesso Aberto de Portugal (RCAAP), EBSCO host (CINAHL e Medline) e Web of Science, utilizando estratégias de pesquisa adaptadas a cada base de dados. Resultados e discussão: A evidência científica demonstra a importância das equipas multidisciplinares na abordagem à pessoa com ferida complexa. Conclusão: Tratando-se de um processo complexo, é fundamental ter em consideração todos os elementos que influenciam e atrasam a cicatrização, para se conseguir melhorar os resultados no tratamento e nos custos associados.
- Importância da nutrição na cicatrização de feridas: uma scoping reviewPublication . Método: Scoping Review de artigos completos e de acesso livre publicados entre 2010 e 2020 nas bases de dados Google Académico, EBSCOhost e PubMed. Resultados e discussão: Todos os artigos analisados explanam a importância da nutrição no processo de cicatrização de feridas nomeadamente na redução do tempo de cicatrização. Conclusão: Parece haver consenso nos artigos analisados sobre a relação direta entre a nutrição e a cicatrização de feridas, quer ao nível do tratamento quer da prevenção.
- Integrating factors associated with complex wound healing into a mobile application: findings from a cohort studyPublication . Marques, Raquel; Lopes, Marcos Venícios Oliveira de; Neves‐Amado, João Daniel; Ramos, Paulo Alexandre Silva; Sá, Luís Octávio de; Oliveira, Irene Maria Silva da; Amado, João Manuel Costa da; Vasconcelos, Maria João Medeiros de; Salgado, Pedro Miguel Faria; Alves, Paulo Jorge PereiraComplex, chronic or hard-to-heal wounds are a prevalent health problem worldwide, with significant physical, psychological and social consequences. This study aims to identify factors associated with the healing process of these wounds and develop a mobile application for wound care that incorporates these factors. A prospective multicentre cohort study was conducted in nine health units in Portugal, involving data collection through a mobile application by nurses from April to October 2022. The study followed 46 patients with 57 wounds for up to 5 weeks, conducting six evaluations. Healing time was the main outcome measure, analysed using the Mann–Whitney test and three Cox regression models to calculate risk ratios. The study sample comprised various wound types, with pressure ulcers being the most common (61.4%), followed by venous leg ulcers (17.5%) and diabetic foot ulcers (8.8%). Factors that were found to impair the wound healing process included chronic kidney disease (U = 13.50; p = 0.046), obesity (U = 18.0; p = 0.021), non-adherence to treatment (U = 1.0; p = 0.029) and interference of the wound with daily routines (U = 11.0; p = 0.028). Risk factors for delayed healing over time were identified as bone involvement (RR 3.91; p < 0.001), presence of odour (RR 3.36; p = 0.007), presence of neuropathy (RR 2.49; p = 0.002), use of anti-inflammatory drugs (RR 2.45; p = 0.011), stalled wound (RR 2.26; p = 0.022), greater width (RR 2.03; p = 0.002), greater depth (RR 1.72; p = 0.036) and a high score on the healing scale (RR 1.21; p = 0.001). Integrating the identified risk factors for delayed healing into the assessment of patients and incorporating them into a mobile application can enhance decision-making in wound care.
