- Climate change prevention through community actions and empowerment: a scoping reviewPublication . Costa, Maria João Salvador; Leitão, Alexandra; Silva, Rosa; Monteiro, Vanessa; Melo, PedroAs society tries to tackle climate change around the globe, communities need to reduce its impact on human health. The purpose of this review is to identify key stakeholders involved in mitigating and adapting to climate change, as well as the type and characteristics of community empowerment actions implemented so far to address the problem. Published and unpublished studies from January 2005 to March 2022 in English and Portuguese were included in this review. The search, conducted on PubMed, CINAHL, Scopus, MEDLINE, Scopus, Web of Science, SciELO, and RCAAP (Repositório Científico de Acesso Aberto de Portugal), followed a three-step search strategy. Data extraction was performed by two independent reviewers, using an extraction tool specifically designed for the review questions. Twenty-seven studies were eligible for inclusion: six used interviews as a qualitative method, three were systematic reviews, three were case study analyses, three used surveys and questionnaires as quantitative methods, two used integrative baseline reviews, and three utilized a process model design. Six studies targeted local, public and private stakeholders. Community settings were the context target of fifteen studies, whereas twelve specifically referred to urban settings. Seven types of community actions were acknowledged across the globe, characterised as hybrid interventions and referring to the leading stakeholders: local governments, non-governmental organizations, civil society, universities, public health, and private sectors.
- Community-dwelling elderly: ineffective management of drug regimen and risk fallPublication . Costa, Tânia; Pinto, Catarina; Afonso, Diogo; Almeida, Armando; Silva, Rosa; Coelho, Patrícia; Alves, PauloIntroduction: Drugs intake is common in the elderly, but pharmacodynamics/pharmacokinetic changes associated with multiple pathologiy amplifys the drug interactions and adverted effects. Objectives: Evaluate the management of drug regimen for the elderly residing at home (N=37), relating it to falls. Methods: Data collection was carried out by semi-structured interview (using instruments validated for the Portuguese reality) followed by descriptive/inferential analysis. Insured informed consent. Results: Average of drug ingestion was 6 drugs/day(min:2 /max:6) and 7 pills/day (min:2 /max:15),. A total of 9 prescribers were described: family physician (N=32), the hospital physician (N=26), the pharmacist (N = 6) and the elderly (N = 6). The reasons why we found that 25 elderly do not meet treatment as prescribed, was due to forgetfulness (68 %) and switching medications (23%). The management of medication regimen presents statistically significant differences compared to moderate/high risk of falling (þ=0.030), dependence in activities of daily living (þ=0.008), geriatric depression (þ=0.022), number of pills/day (þ=0.006) and number of diferent drugs (þ=0.009). The risk of falling, in other way, is related to cognitive impairment (þ=0.032), dependence in instrumental activities of daily living (þ=0.005) and number of pills/day (þ=0.043). Conclusion: Data seem indicate that there is a proportional relationship between fall risk and ineffective management of medication regimen. Considering the prevalence of falls in the elderly and it’s repercussions, highlights the need to implement nursing care of proximity/partnership, but also, integrals and integrated in both diagnoses, especially in the context of primary health care.

- Delirium in the hospitalized patient: a Portuguese contextPublication . Silva, Rosa; Sousa, Joana; Neves, HugoIntroduction.– ‘‘Delirium’’, also known as Acute Confusion (AC), is associated to significant incidence rates and weak outcomes, with great impact in the patient. It is a complex syndrome frequently subdiagnosed by the interdisciplinary team. Due to such problems our purpose in this work is to: translate and validate to Portuguese a scale capable of diagnosing the syndrome; identify the frequency of patients and their characteristics in a university hospital; perform a patient’s follow-up one month after the evaluation of the ‘‘delirium’’ episode.

- Dependency in selfcare in the institutionalized elder: A descriptive studyPublication . Sousa, Joana; Silva, Rosa; Ferreira, Bruno; Neves, BrunoIntroduction.– Autonomy in the performance of daily living activities (DLA) are of extreme importance to the elder’s life. On pair with aging, the institutionalized elder, suffers a transition process from the changes of context from home to institution. This fact increases the elder’s dependency on self-care. By so, it is important to study the dependency degree in self-care in institutionalized elders, in order to rethink interventions to answer context changes and improve transition. Objective.– Identify the dependency degree in the institutionalized elder; explore the use of support products (SP) in self care and existance of structural barriers. Methods.– An exploratory-descriptive study, with a nonprobabilistic convinience sample was developed in two nursing homes. Instrument used was Hernâni’s Form (2009). Results.– In a total of 84 elders, averaging 87 years, with a minimum of 68 and maximum of 102 years, 45% widows, 17% analphabets, being the majority women (84%), 39% refered dependency of others as the motive for institutionalization. Bath self-care dependency was the highest self-care with dependency of the evaluated, with 79%presentedsomedegree of dependency. Eating self-careshowed the least degree of dependency (43%). Structural barriers found were steps with ramp, the SPs found were almost inexistence, being lateral support bars the most common. Conclusions.– The dependency degree of some self-cares lack interventions that target autonomy. As so, we consider that nurses should evaluate the elder’s potential for learning of new skills in order to reconstruct the process of autonomy, decreasing levels of dependency and increasing quality life.
