- From mouth to mind : molecular insights into oral health and neurodepressionPublication . La Vecchia, Angelica; Duarte, Ana Sofia Direito dos Santos; Silva, Vanessa Maria Barroso dos SantosObjective: This thesis aims to explore the molecular mechanisms involved in the bidirectional relationship between oral health-particularly periodontitis-and depressive disorders. The main objective is to examine how inflammatory and immunological alterations associated with periodontitis may contribute to the onset and worsening of depressive symptoms, and, conversely, how molecular imbalances typical of depression may negatively affect oral health. Materials and Methods: A systematic review of the scientific literature was conducted using the databases PubMed, Web of Science, and Scopus, in accordance with PRISMA and JBI guidelines. Results: The analysis of 27 studies revealed a central role for pro-inflammatory cytokines as key biomarkers in the molecular mechanisms underlying this interrelationship. In the context of mental disorders, these molecules are implicated in alterations such as increased blood-brain barrier permeability and dysfunctions of the hypothalamic-pituitary-adrenal (HPA) axis-both of which are associated with cerebral dysregulation and the exacerbation of depressive states. In the field of oral health, the same cytokines influence the progression of periodontitis through changes in microbiota composition, hormonal levels, and local enzymatic activity. These findings highlight how the oral and neuropsychological systems are closely interconnected through shared biological pathways. Conclusion: There is strong evidence that specific inflammatory and molecular pathways are simultaneously altered in patients with both depression and compromised oral health, compared to depressed individuals with preserved oral conditions. This interdependence underscores the need for an integrated clinical approach that considers the individual as a complex and unified psychophysical system. Mens sana in corp(ore) sano.
